桥本甲状腺炎患者血清内脂素与白细胞介素-6和甲状腺功能的相关性研究

摘　　要：目的观察桥本甲状腺炎(HT)患者血清内脂素的变化及其与白细胞介素-6(IL-6)、甲状腺功能的相关性。方法选取2014年3-10月在该院诊治的HT患者91例为实验组,其中HT甲亢31例,HT甲减32例,HT甲状腺功能正常28例。另选同期健康体检人群30例为正常对照组。测定各组研究对象的体重指数(BMI)、血清游离三碘甲腺原氨酸(FT3)、游离甲状腺素(FT4)、三碘甲腺原氨酸(TT3)、甲状腺素(TT4)、促甲状腺素(TSH)、抗甲状腺球蛋白抗体(TGAb)、抗甲状腺过氧化物酶抗体(TPOAb)、内脂素和IL-6水平。分析各组内脂素的变化及其与IL-6、甲状腺功能等指标的相关性。结果与对照组比较,HT甲亢组和HT甲减组内脂素、IL-6水平均明显升高(P<0.05),而HT甲状腺正常组则无明显差异(P>0.05)。HT甲亢组的内脂素水平与IL-6、FT3、TT3、TT4正相关,与BMI、TSH负相关。HT甲减组的内脂素水平与IL-6、TSH正相关,与TT4负相关。结论 HT甲亢和甲减患者内脂素、IL-6水平均明显升高,并与甲状腺功能相关。【Objective】To investigate the change of plasma visfatin concentration in patients with Hashimoto’s thyroiditis（HT）, and analyze the relationship among plasma visfatin, IL-6 and thyroid function. 【Methods】From March to October 2014, 91 cases of patients with Hashimoto’s thyroiditis including 31 cases of hyperthyroidism, 32 cases of hypothyroidism and 28 cases of euthyroidism were chosen to experimental group, while 30 euthyroid healthy check-up cases were into control group. Fasting plasma visfatin, IL-6, BMI, FT3, FT4, TT3, TT4, TSH, TGAb and TPOAb were measured. The relationships among plasma visfatin, IL-6 and thyroid function were analyzed.【Results】Compared with the control group, the fasting serum levels of visfatin and IL-6 were significantly increased in the hyperthyroidism group and the hypothyroidism group（P 〈 0.05）, while there were no differences in the euthyroidism group（P 〉 0.05）. In the hyperthyroidism group, visfatin positively correlated with IL-6, FT3, TT3 and TT4, while negatively correlated with BMI and TSH. But in the hypothyroidism group, visfatin positively correlated with IL-6 and TSH, while negatively correlated with TT4. 【Conclusions】The fasting serum levels of visfatin and IL-6 significantly increase in the hyperthyroidism and hypothyroidism patients of Hashimoto’s thyroiditis, which are correlated with thyroid function.
