湿润烧伤膏与磺胺嘧啶银在深度烧伤保留变性真皮自体皮移植术中的应用比较

摘　　要：目的比较研究湿润烧伤膏与磺胺嘧啶银在磨痂保留变性真皮自体皮移植术治疗深度烧伤的临床疗效。方法选取2012年5月-2013年12月期间该院收治的90例深Ⅱ度和深Ⅲ度烧伤患者纳入研究,给予磨痂保留变性真皮自体皮移植术,分为接受湿润烧伤膏外用的观察组和磺胺嘧啶银外用的对照组,比较两组患者的创面愈合情况、机体炎症反应和应激反应。结果 1创面愈合情况:观察组患者创面愈合等级优于对照组,创面愈合时间(19.3±2.4)d,发生创面感染和瘢痕愈合(4.44%,13.33%)明显低于对照组;2炎症应激反应:观察组肾上腺素(102.3±14.3)ng/m L、去甲肾上腺素(98.4±14.2)ng/m L、肾素水平(2.7±0.5)pg/m L以及IL-1、TNF-α水平(46.6±1.1)μg/L,3.4±0.6)μg/L均明显低于对照组。结论磨痂保留变性真皮自体皮移植术联合湿润烧伤膏治疗有助于促进创面愈合,预防创面感染和瘢痕愈合,缓解炎症应激反应。【Objective】To compare the clinical curative effect of Moisture Burn Ointment（MEBO） and Silver Sulfadiazine in the repair of deep burn with dermabrasion reserved denatured auto skin grafting. 【Methods】Ninety cases with deep second degree and third degree burn from May 2012-to December 2013 were enrolled and given dermabrasion reserved denatured dermis autologous skin graft treatment. Then patients were divided into observation group given Moisture Burn Ointment and control group given Silver Sulfadiazine. Wound healing condition, inflammatory response and stress response of both groups were compared. 【Results】1 Wound healing condition： in the observation group wound healing was significantly better than that in the control group; the wound healing time was（19.3 ± 2.4） d, the wound infection and scar healing rates（4.44% and 13.33%, respectively） were significantly lower than those in the control group; 2Inflammatory and stress response： adrenaline [（102.3 ± 14.3） ng/ml], noradrenaline[（98.4 ± 14.2） ng/ml], renin [（2.7 ± 0.5） pg/ml], IL-1 [（46.6 ± 1.1） μg/L] and TNF-a [（3.4 ± 0.6） μg/L] levels in the observation group were significantly lower than those in the control group. 【Conclusions】Dermabrasion reserved denatured dermis autologous skin grafting combined with MEBO is an ideal therapy in treating deep burn because it can promote wound healing, prevent wound infection and scar healing, and alleviate inflammatory and stress responses.
