摘　　要：目的构建并实施医院-社区压疮延伸服务模式,使三级医院的优良资源有效下沉至社区,提升社区压疮护理的能力。方法 2013年10月,同济大学附属杨浦医院通过专科技能培训、远程会诊、双向转诊等方式构建并实施医院-社区压疮延伸服务模式,比较模式实施前后的患者院内压疮发生率及Ⅰ、Ⅱ、Ⅲ期压疮治愈率。结果实施延伸服务模式后,4家社区医院的患者院内压疮发生率均明显低于实施前,各期压疮治愈率均明显高于实施前,差异有统计学意义(均P<0.05)。结论压疮延伸服务模式有利于提升社区医院的压疮护理能力,缓解三级医院床位紧张的现状,有效降低压疮的发生率、提高压疮的治愈率。Objective To construct and implement the hospital-community extended care service mode of pressure ulcer,in order to make the excellent resources of Class III hospitals can be shared with the community,and improve the quality of community hospital care of pressure ulcer.Methods In October 2013,the Yangpu hospital construct and implement the hospital-community pressure ulcer extended service model through the specialty skills training,remote consultation and two-way referral consultation.The incidence of pressure ulcers in the hospital and the cure rate of the patients with I,II,III level pressure ulcer before and after the implementation were compared.ResultsAfter the implementation of extended service model,the incidence of pressure ulcers in 4community hospital was decreased compared with before,and the cure rate of the patients with I,II,III level of pressure ulcer were significantly higher than before the implementation of the mode（all P〈0.05）.Conclusion The construction of the extended care service of pressure ulcer could improve the quality of community hospital care,relieve the tense situation of Class III hospital resource,reduce the incidence of pressure sores and improve the cure rate of pressure ulcer effectively.
