摘 要:目的分析不典型乳腺癌的不同病理类型在计算机断层扫描(CT)和核磁共振(MRI)影像下的特征,为影像学诊断不典型乳腺癌提供指导。方法选取经过手术病理确诊的不典型乳腺癌患者60例,收集患者术前2 w内进行的CT和MRI检查结果,分析CT和MRI影像学特征表现与病理结果的相关性,并计算两者对不典型乳腺癌诊断的灵敏度。结果本组60例中,非浸润性乳腺癌12例,浸润性乳腺癌48例;导管内癌40例,小叶型癌和其他类型癌症各10例。CT共检出肿块48例,钙化灶40个,边缘毛刺征31例,腋窝淋巴结肿大8例,其影像学特征在肿瘤的浸润程度和病理类型上的分布无统计学差异。MRI检查出肿块52例,钙化灶10例,边缘毛刺征51例,腋窝淋巴结肿大10例,其影像学特征在肿瘤浸润程度上的分布无差异,但在不同病理类型上的分布存在统计学差异。CT诊断灵敏度为80.0%,MRI诊断灵敏度为86.7%,二者联合诊断的灵敏度为91.7%。结论不同病理类型的不典型乳腺癌在MRI影像上的影像特征不同,CT联合MRI诊断不典型乳腺癌灵敏度比单一检查诊断灵敏度高。Objective To analyze the characteristics of different pathologic types of non typical breast cancer on computerized tomography (CT) and magnetic resonance imaging, and to provide guidance for the imaging diagnosis of untypical breast cancer. Methods Sixty patients who were pathologically diagnosed as untypical breast cancer by operation were selected. The imaging results of CT and MRI within two weeks before the operation were collected in order to analyze the correlations between the characteristics of CT and MRI and the pathological results. The sensitivity of both methods to the diagnosis of untypical breast cancer was calculated. Results In all the 60 cases, there were 12 cases of non invasive breast cancer, 48 ones of invasive breast cancer, 40 ones of intraductal cancer, and 10 ones of lobular cancer and other types of cancer. CT showed 48 masses, 40 calcified foci, 31 burrs on edges, and 8 axillary lymphadenectasis. There was no significant difference between the distribution of imaging characteristics in invasive degrees and pathological types. MRI showed 52 masses, 10 calcified foci, 51 burrs on edges, and 10 axillary lymphadenectasis. There was no significant difference in the distribution of imaging characteristics in invasive degrees, but the differences in the distribution in different pathological types were significant. The sensitivity of CT was 80.0%, while that of MRI was 86.7%. The sensitivity of the combination was 91.7%. Conclusion Different pathological types of untypical breast cancer have different imaging characteristics on MRI. The combination of CT and MRI in the diagnosis of untypical breast cancer has a higher sensitivity than the single examination method.
