LCP和PFNA手术治疗股骨转子间骨折临床疗效比较

摘　　要：目的通过回顾性研究,比较股骨近端解剖型锁定加压钢板（locking compression plate,LCP）和股骨近端螺旋刀片抗旋髓内钉（proximal femoral nail antirotation blade,PFNA）在治疗股骨转子间骨折中的应用和临床疗效。方法回顾性分析采用LCP钢板和PFNA治疗股骨转子间骨折60例,分成LCP组和PFNA组,LCP组是早期使用LCP内固定治疗的患者（30例）,PFNA组是后期采用PFNA治疗的患者（30例）。对比分析两组手术时间、术中失血量及手术优秀率。结果本组所有患者完成10~15个月随访,平均12.8个月。PFNA组优秀率（83.33%）,高于LCP组（60.00%）,差异具有显著性（P〈0.05）;PFNA组手术平均用时及术中出血量均少于LCP组,PFNA组未发现明显并发症。结论 LCP和PFNA钢板内固定术,均是治疗股骨转子间骨折安全有效的方法,PFNA内固定治疗股骨转子间骨折患者疗效更佳、手术时间更短、术中出血量更少且安全,值得临床进一步推广应用。Objective To compare anatomical proximal femoral locking compression plate（LCP） and proximal femoral nail antirotation blade（PFNA）in the treatment of intertrochanteric fractures applications and clinical efficacy.Methods A retrospective analysis was performed using comparison between LCP and PFNA treatment of intertrochanteric fractures of 60 cases,which were divided into LCP group and PFNA group. LCP group was the patients（30 cases）previausly underwent LCP therapy. PFNA group was the patients（30 cases）afterwards underwent PFNA therapy. Comparative analysis of operative time,intraoperative blood loss and surgical excellent rates in two groups.Results All patients in PFNA group received 10 to 15 months follow- up,an average of 12. 8 months. PFNA group excellent rate was 83.33%,higher than the excellent rate of LCP group which was 60%. The difference was significant（P〈0.05）. The average time of surgery and blood loss volume of PFNA group are all less than those of LCP group. In PFNA group no significant complications were found. Conclusions LCP and PFNA fixation were all safe and effective method to treat intertrochanteric fractures. PFNA internal fixation has better efficacy,shorter operative time,less blood loss,but also safe and worthy of further clinical application.
