摘 要:目的探讨经阴道子宫输卵管四维超声造影(TVS 4D-Hy Co Sy)在临床输卵管通畅性评价的应用价值。方法选择不孕症患者160例,进行TVS 4D-Hy Co Sy检查,通过四维彩超多平面重建技术实时动态观察造影剂在子宫宫腔及输卵管腔内流动过程,判断输卵管的通畅性,其中49例同时进行子宫输卵管碘油造影(HSG),72例进行腹腔镜检查。结果 160例患者共计316条输卵管,经超声造影后显示输卵管畅通209条,输卵管通而不畅43条,输卵管不通64条。49例HSG检查总体符合率97.96%,72例腹腔镜检查总体符合率91.43%。结论经阴道子宫输卵管四维超声造影可动态检测子宫宫腔和输卵管的解剖结构、管腔真实形态,可有效判定输卵管的通畅,可为临床不孕症患者的准确诊断提供一种新型、简易有效的方法,具有较高的推广应用前景。Objective To explore the clinical value of fallopian tubes patency assessment by using transvaginal 4-dimensional hysterosalpingo-contrast sonography(4D-Hy Co Sy). Methods 160 cases of infertility were selected. All cases received TVS 4D-Hy Co Sy inspection, and real-time assessment of tubal patency was conducted by monitoring motion of contrast agent in the endometrial cavity and tuballumen. In addition, 49 cases received hysterosalpingography(HSG), and 72 cases received laparoscopy inspection. Results Among 160cases(316 strips of fallopian tubes), the ultrasound contrast showed 209 strips of unblocked, 43 strips of partially obstructed and 64 strips of obstructed. The overall coincidence rate of 49 cases of HSG inspection was 97.96%, and the overall coincidence rate of 72 cases of laparoscopy inspection was 91.43%. Conclusions Vaginal tubal anatomy of four-dimensional ultrasound imaging can dynamically detect uterine uterus and fallopian tube lumen true form, which can be a new, simple and effective method for clinical diagnosis of infertility.
