NIPPV治疗老年AECOPD合并Ⅱ型呼吸衰竭的临床疗效评价

摘　　要：目的探讨分析使用NIPPV(无创正压通气)治疗AECOPD(慢性阻塞性肺疾病急性加重期)合并Ⅱ型呼吸衰竭的老年患者的临床疗效。方法将我院80例老年AECOPD合并Ⅱ型呼吸衰竭患者随机分为两组各40例。在给予相同基础治疗的同时,治疗组给予NIPPV辅助通气,对照组给予传统有创通气治疗,比较两组的临床疗效。结果两组患者治疗后24 h及48 h平均动脉压、APACHEⅡ评分、p H值、Pa CO2及Pa O2等指标均有好转(P<0.05),且治疗组患者情况改善明显好于对照组患者(P<0.05)。结论 NIPPV对老年AECOPD合并Ⅱ型呼吸衰竭患者有明显的治疗效果。Objective To investigate the clinical efficacy of non-invasive positive pressure ventilation（NIPPV） for treatment of senior AECOPD patients complicated with type Ⅱ respiratory failure. Methods 80 senior patients diagnosed with AECOPD complicated with typeⅡ respiratory failure were randomly divided into two groups equally. On the basis of basic treatment, patients in the treatment group were given NIPPV, while patients in the control group were given routine invasive ventilation. The clinical effects of two groups were compared.Results The mean arterial pressure, APACHEⅡ score, p H, Pa CO2 and Pa O2 of both groups improved significantly after treatment, and the treatment group was superior to the control group after treatment（P 0.05）. Conclusions NIPPV has significant curative effect for senior patients diagnosed with AECOPD complicated with type Ⅱ respiratory failure.
