摘　　要：目的:观察针刀治疗对中轴型强直性脊柱炎患者外周血中P物质、IL-6、IL-2、TNF-a表达的影响并探讨其治疗机理。方法:将80例患者按随机数字表法分为治疗组和对照组各40例,分别采用针刀和药物治疗观察疗效及相关指标变化。结果:2组患者治疗后BASDAI、BASFI、脊柱疼痛Likert四级积分、P物质、IL-6、IL-2和TNF-a指标均降低,且治疗组优于对照组,差异有统计学意义。结论:针刀治疗中轴型AS的疗效显著,可能与其抑制IL-6、IL-2、TNF-a、P物质的表达有关。Objective： To observe the effect of acupotome on the expression of substance P,IL-6,IL-2 and TNF-a of ankylosing spondylitis（ AS） patients,and to explore the mechanism of AS treatments with acupotome. Methods： 80 AS patients were randomly allocated to treatment group and control group,and treated with acupuncture and drug. To observe the changes of clinical and laboratory indexes. Results： By analyzing BASDAI,BASFI,spine ache Likert level 4 integral,P substance,IL-6,IL-2 and TNF-a of the two groups patients before and after treatment. After treatment,all indexes were measured is lower in both groups. And each index of treatment group is better than control group,there was a significant difference（ P 0. 05）. Conclusion： The efficacy of acupuncture therapy of AS may be related to inhibit immunocompetent cells to secrete IL-6,IL-2,TNF-a and reduce inflammatory mediators substance P.
