摘 要:目的:探讨MRI用于骨坏死的诊断及临床应用价值.方法:对45例骨坏死患者的患病部位行MRI检查.结果:45例患者中30例为股骨头坏死,共累及49个股骨头,49个病变股骨头的T1WI均呈低信号,T2WI出现高信号带.其中有2例股骨头上区T1WI低信号呈现小圆形,T2WI呈小环状,周边为低信号环,中心是高信号.15例患者为膝关节坏死,共累及21个膝关节,发病位置多位于股骨远端和胫骨平台的软骨下骨,内侧踝发病率最高(共12例),可能与该部位为膝关节主要负重面有关.结论:MRI因其多序列、多平面成像能力能够早期诊断骨坏死等病理改变,对骨坏死的早期治疗有着积极作用.AIM: To evaluate the value of MRI in diagnosing bone necrosis and its clinical application. METHODS: The dis- eased parts of 45 patients with osteonecrosis were examined by MRI. RESULTS : Among the 45 patients, there were 30 cases of femoral head necrosis, involving a total of 49 femoral heads, and the T1WI showed low signal, T2WI showed high signal. Among them, T1WI low signal in upper femoral head present as small round area, T2WI present as a small ring, cireum was the low signal ring, and the center showed high signal. And 15 patients were diagnosed as osteoneerosis of the knee, involving 21 knee joints. The diseased areas were located at the distal femur and tib- ial subehondral bone. The medial ankle had the highest incidence (12 eases), and it may be related to that the medial ankle was the main loading surface of knee joint. CONCLUSION: The MRI could diagnose the bone necrosis early and other pathological changes because of its multi series and multi plane imaging capability, and it plays a positive role in early treatment of bone necrosis.
