超声二维斑点追踪技术评价无症状饮酒者左心房功能的变化

摘　　要：目的应用二维斑点追踪成像(2D-STE)技术评价无症状饮酒者随着饮酒时间因素改变对左心房功能的影响。方法选取30例体检健康不饮酒男性作为A组(对照组)。92例男性饮酒者根据饮酒时间长短分为3组:B组(轻度组)、C组(中度组)、D组(重度组)。2D-STE测量左心房最大容积(LAVmax)、最小容积(LAVmin)、左心房收缩前容积(LAVp)、左心房被动射血分数(LAPEF)、左心房主动射血分数(LAAEF)、心室收缩期左心房整体应变率(SRs)、心室舒张早期左心房整体应变率(SRe)及心室舒张晚期左心房整体应变率(SRa)。结果 B组所有指标与A组比较无统计学意义(均P>0.05)。C组LAVmin、LAVmax、LAVp、LAAEF及SRa与A、B组比较明显增大(均P<0.05),LAPEF、SRs及SRe明显减少(均P<0.05)。D组LVDd、LVDs、IVSTd、PWd、LAVmin、LAVmax及LAVp与A、B、C组比较明显增大(均P<0.05),LVEF、FS、LAPEF、SRs、SRe、LAAEF及SRa明显减少(均P<0.05)。结论左心房功能的损害会根据饮酒时间的改变而改变,2D-STE技术能够有效评价无症状饮酒者左心房功能的变化情况。Objective To assess left atrial function changes among alcoholics related to the duration of heavy drinking by 2D speckle tracking echocardiography（2D-STE）.Methods 30 health male adults without drinking history were used as the group A（control group）.92 male asymptomatic alcoholics were divided into three groups：group B：mild group.group C：moderate group.group D：severe group.2D-STE：left atrial maximum volume（LAVmax）,left atrial minimum volume（LAVmin）,left atrial pre-atrial contraction volume（LAVp）,left atrial passive ejection fraction （LAPEF）,active ejection fraction （LAAEF）,global peak atrial strain rate of systolic,early and late diastolic（SRs,SRe,and SRa） of left atrial.Results All parameters were no significant difference between group B and group A（all P＞0.05）.LAVmin,LAVmax,LAVp,LAAEF and SRa increased significantly through the comparison of group C with group A and group B（all P＜0.05）,while LAPEF,SRs and SRe decreased significantly（all P＜0.05）.LVDd,LVDs,IVSTd,PWd,LAVmin,LAVmax and LAVp increased significantly through the comparison of group D with group A,group B and group C（all P＜0.05）,while LVEF,FS,LAPEF,SRs,SRe,LAAEF and SRa decreased significantly（all P＜0.05）.Conclusions 2D-STE could be a method to effectively assess left atrial function in asymptomatic alcoholics.
