超声引导下无水乙醇治疗继发性甲状旁腺功能亢进的应用价值

摘　　要：目的评估超声引导下医用无水乙醇治疗继发性甲状旁腺功能亢进的治疗效果和应用价值。方法比较分析56例慢性肾功能衰竭继发甲状旁腺功能亢进患者,经超声引导无水乙醇治疗前后甲状旁腺超声图像和血清甲状旁腺激素(PTH)水平的变化,评价其治疗效果。结果无水乙醇穿刺介入治疗前后对比甲状旁腺回声不均匀增强,血流信号明显减少。甲状旁腺腺体积由(2.32±0.64)cm3缩小为(2.15±0.53)cm3,但差异无统计学意义(t=1.65,P>0.05);血清PTH水平由(987±95.2)ng/L明显下降为(434±56.6)ng/L,差异有统计学意义(t=5.96,P<0.01)。结论超声引导下无水乙醇治疗继发性甲状旁腺功能亢进疗效确切,治疗前后超声图像及血PTH的变化可作为观察疗效的重要指标,具有重要临床应用价值。Objective To evaluate therapeutic effect and application value of the therapy of secondary hyperparathyroidism with ultrasound guided percutaneous injection of medical anhydrous ethanol.Methods Fifty-six patients of secondary hyperparathyroidism were comparatively analyzed in the sonography and serum parathormone （PTH） before and after interventional therapy to evaluate therapeutic effect.Results After injection of ethanol the echoes unevenly enhanced and the blood flow obviously reduced.Gland shrinked （2.32 ± 0.64） cm3,（2.15 ± 0.53） cm3,t =1.65,P ＞0.05,the levels of PTH were significantly decreased（987 ± 95.2） ng/L,（434 ± 56.6） ng/L,t =5.96,P＜0.01.Conclusions The therapy of secondary hyperparathyroidism with ultrasound guided percutaneous injection of ethanol in chronic dialysis patients was exactly effected and recommended.The sonography and serum PTH are important indicators to estimate the effects.It has an important clincial application value.
