北京大兴地区变应性鼻炎吸入性变应原谱分析

摘　　要：目的调查北京大兴地区变应性鼻炎（AR）患者吸入组变应原分布。方法采用敏筛定量变应原检测系统（allergyscreen）测定1200例变应性鼻炎患者血清中吸入组变应原的slgE，将AR患者按年龄分为2组：A组3～18岁，B组18N82岁，并比较2组吸入组变应原的分布状况。结果①不同年龄组患者对不同变应原反应呈阳性的例数分析显示，A组（3N18岁）首要变应原是混合真菌（61．26％），其次为户尘螨（57．47％）、屋尘（38．32％）等；B组（18～82岁）首要变应原是户尘螨（67．45％），其次为屋尘（58．48％）、混合真菌（35．72％）等。②各类变应原slgE≥III级阳性前3位的依次为A组（3N18岁）混合真菌（62．89％），户尘螨（56．4l％），蒿（52．13％）；B组（18～82岁）户尘螨（60．74％），矮豚草（48．33％），蜂螂（41-38％）。③不同性别和年龄患者双重变应原阳性依次是户尘螨＋混合真菌，户尘螨＋屋尘，户尘螨＋蟑螂。结论户尘螨，屋尘，混合真菌是北京大兴地区AR的主要变应原，不同年龄阶段的患者致病变应原有所不同。ObjectiveTo investigate the inhalant allergen spectrum in patients with allergic rhinitis in Beijing Daxing District.MethodsA sensitive quantitative allergen screen system(Allergy Screen)was used in 1200 patients with allergic rhinitis to test the sIgE of inhalant allergens. The patients were divided into two groups according to their ages,group A(3~18 years old)and B(18~82 years old). And the distribution of inhaled allergens of the two groups was compared.ResultThe most common allergen found in group A was mixed fungi(61.26%),followed by house dust mite(57.47%)and house dust(38.32%)etc.The most common allergen found in group B was house dust mite (67.45%),house dust(58.48%),followed by and mixed fungi(35.72%)etc.The top three allergens whose sIgE≥III were mixed fungi(62.89%),house dust mite(56.41%) and mugwort(52.13%)in group A;and house dust mite (60.74%),dwarf ragweed(48.33%)and cockroach (41.38%)in group B. As for dual positive allergens in all patients,the most common allergen combination was mixed fungi + house dust mite,house dust mite + house dust and house dust mite + cockroach.ConclusionThe house dust mite,house dust and mixed fungi were the most important three aeroallergens in Beijing Daxing District. However,the pathogenic allergens were different in patients with different ages.
