摘 要:目的探讨宫颈细胞学筛查中新柏玻片扫描分析影像系统(TIS)辅助阅片系统的价值。方法回顾性分析比较宫颈液基涂片60147例人工阅片和52873例TIS阅片平均时间、上皮病变的检出率及细胞学和组织学的符合率。结果相较于人工阅片,TIS平均阅片速度提高90%。鳞状上皮病变总检出率及非典型鳞状上皮细胞(ASC)和低级别鳞状上皮内病变(LSIL)检出率提高,高级别鳞状上皮内病变(HSIL)和非典型腺上皮细胞(AGC)检出率差异不显著。ASC/SIL比值下降。鳞状上皮内病变细胞学和组织学符合率高。结论 TIS作为一种新的辅助阅片系统,有助于提高工作效率,对鳞状上皮病变有较高检出率。对腺细胞病变的检测,有待探讨。Objective To discuss the value of Thinprep imaging system in cervical cytology screening.Methods A total of 60147 manually screened Thinprep Pap slides were compared with 52873 imaged slides for average screening time,diagnostic rates of epithelial lesion and the correlation of cytopathology with histopathology.Results Compared with manual screening,TIS showed a 90% increase in productivity.As for detection rate of overall positivity,ASC and LSIL were increased but those of HSIL and AGC were no significant difference.ASC to SIL ratio was decreased.Coincidence rate between cytopathology and histopathology was high in detection of squamous intraepithelial lesion with TIS.Conclusion As a new assisted imaging system,TIS contributes to improve work efficiency and increase the overall positive detection rate of squamous epithelial lesion.But its detection efficiency of glandular epithelia still remaines to be further studied.
