男性年龄对体外授精-胚胎移植妊娠结局的影响

摘　　要：目的：探讨中国男性年龄对体外授精-胚胎移植（IVF-ET）妊娠结局的影响。方法：回顾性分析广州医科大学附属第三医院生殖医学中心2009年1月1日至2013年10月31日的IVF周期。女方按年龄分为3组：〈30岁、30~34岁、35~38岁,在女方年龄分层基础上按男方年龄再分为6组：〈30岁、30~32岁、33~35岁、36~38岁、39~41岁、≥42岁。比较不同男性年龄组的种植率、妊娠率、流产率、活产率。结果：不同男性年龄组,女方的基础血E2、FSH、h CG日内膜厚度、取卵数、胚胎移植天数无统计学差异（P均〉0.05）。不同男性年龄组（〈30、30~32、33~35、36~38、39~41、≥42）种植率呈下降趋势：女方年龄〈30岁时,种植率分别为41.1%、42.0%、39.5%、31.3%、40.7%、48.6%（P〈0.05）;女方年龄30~34岁时,种植率分别为40.3%、36.4%、35.1%、35.3%、29.4%、37.3%（P〈0.05）;女方年龄35~38岁时,种植率分别为48.2%、17.8%、25.3%、23.5%、22.1%、23.8%（P〈0.05）。女方年龄30~34岁时,男方年龄≥39岁组流产率显著高于30~32岁、33~35岁2组（P〈0.05）。女性年龄〈30岁组及35~38岁组,不同男性年龄组流产率无统计学差异（P〉0.05）。不同男性年龄组妊娠率、活产率无统计学差异（P〉0.05）。结论：男性年龄对IVF妊娠率及活产率无显著影响,对种植率、流产率有一定的影响。Objective： To investigate the influence of male age on the pregnancy outcomes of in vitro fertilization-embryo transfer（ IVF-ET）. Methods： We retrospectively analyzed 7 533 cycles of IVF-ET performed between January 1,2009 and October 31,2013. We divided the samples into three groups according to the female age（ 〈 30,30- 34,and 35- 38 yr）,each again subdivided into six groups based on the male age（ 〈 30,30- 32,33- 35,36- 38,39- 41,and ≥42 yr）. We compared the rates of implantation,pregnancy,miscarriage,and live birth among different age groups. Results： There were no statistically significant differences in basal E2,FSH,endometrium thickness on the day of h CG administration,number of oocytes retrieved,and days of embryo transfer among different male age groups（ P 〉 0. 05）. The implantation rate showed an age-dependent decrease in the 〈 30,30- 32,33- 35,36- 38,39- 41,and ≥42 yr male groups,41. 1,42. 0,39. 5,31. 3,40. 7,and 48. 6% among the women aged 〈 30 years（ P 〈0. 05）,40. 3,36. 4,35. 1,35. 3,29. 4,and 37. 3% among the women aged 30- 34 years（ P 〈 0. 05）,and 48. 2,17. 8,25. 3,23. 5,22. 1,and 23. 8% among the women aged 35- 38 years（ P 〈 0. 05）. The miscarriage rate was significantly higher in the ≥39yr than in the 30- 32 and 33- 35 yr male age groups among the women aged 30- 34 years（ P 〈 0. 05）,but showed no remarkable differences among the other male age groups in the women aged 〈 30 and 35- 38 years（ P 〉 0. 05）. No statistically significant differences were observed in the rates of pregnancy and live birth among different male age groups（ P 〉 0. 05）. Conclusion： Male age has some influence on the rates of implantation and miscarriage but not on the rates of pregnancy and live birth in IVF-ET.
