肛垫缝扎修复术治疗Ⅲ度痔的临床对照研究

摘　　要：目的:探讨肛垫缝扎修复术治疗Ⅲ度痔的疗效。方法:对100例Ⅲ度痔患者随机分组,50例采用肛垫缝扎修复术(治疗组)治疗,50例采用超声多普勒痔动脉结扎术(DG-HAL,对照组)治疗,对两组疗效、并发症及患者满意度进行对比分析。结果:治疗组治愈率为86%,优于对照组的70%,(P<0.05);治疗组治疗费用为3138.09元,对照组为4020.43元(二者比较P<0.05)。随访24个月,治疗组痔脱垂(2.3%)及总体复发率(7.0%)低于对照组(19.0%,23.8%,P<0.05);治疗组患者认为"好"、"很好"者分别为42例(95.4%)、39例(90.7%),对照组分别为37例(88.9%)、32例(76.2%);两组术后疼痛、Wexner评分、继发出血、肛门不适等均具有较好的安全性(P>0.05)。结论:肛垫缝扎修复术与DG-HAL相比,能有效解除Ⅲ度痔病症状,有效、经济、安全,具有更高的患者满意度。Objective To investigate anal cushion suture-fixation operation in the treatment of grade Ⅲ hem-orrhoids.MethodsOne hundred cases of grade III hemorrhoids were randomly divided into treatment group（anal cushion suture-fixation operation,50 cases）and control group（Doppler-guided hemorrhoidal artery ligationDG-HAL）,50 cases.The clinical effects and complications were compared between the two groups.Results Cure rates were 86% in treatment group and 70% in the control（P 0.05）,the cost of treatment group was$3138.09 VS $4020.43 in the control（P 0.05）. Follow-up studies showed recurrences at 24 months afteroperation 7.0% and patients＇ satisfaction 95.4%（the treatment group） VS 23.8% and 88.9%（the control group）（P 0.05）. There were no differences between the two groups（P 0.05）in complications, in postoperative painscores,Wexner scores,bleeding, and the anal discomfort.ConclusionAnal cushion suture-fixation operationis valid in eliminating symptoms induced by grade Ⅲ hemorrhoids.It is effective, economical and with better sat-isfaction than by DG-HAL.
