两种股动脉血管闭合器的临床应用疗效

摘　　要：目的探讨Perclose-proglide及Angio-seal 2种股动脉血管闭合器的特点和使用技巧.方法对比分析经股动脉途径进行介入治疗后分别应用Angio-seal、Perclose-proglide 2种血管闭合器进行股动脉术口闭合止血的190例患者,观察闭合止血术后即刻止血成功率、使用闭合器的操作时间、术肢制动时间、术后24 h内并发症、48 h内术口血肿发生率、血管闭合器的特点和使用技巧.结果股动脉术口闭合成功后,2种血管闭合器的即刻止血成功率均为100%,患者术后24 h内均无出血发生;使用Perclose-proglide血管闭合器的操作时间(6.0±1.21)min,术肢制动时间(8±0.8)h;使用Angio-seal血管闭合器的操作时间(2.0±1.12)min,术肢制动时间(8±0.5)h.2种血管闭合器比较,P<0.05.结论在掌握2种血管闭合器各自的特点和使用技巧后并应用于经过选择的合适患者,均可达到安全、有效止血及无重要血管并发症的目的.Objective To investigate the characteristics and use skills of Perclose- Proglide and Angio- seal femoral artery vascular closure devices. Methods 190 patients who underwent interventional therapy were given Perclose- Proglide and Angio- seal vascular closure devices for femoral artery bleeding wound closure,respectively.The two kinds of femoral artery vascular closure devices were observed and compared in the success rate of closed immediately after hemostasis, operation time, closure intraoperative limb immobilization time, complications within 24 hours after surgery,the incidence of surgical wound hematoma within 48 hours,vascular closure device features and usage tips. Results After success of the femoral artery surgery wound closed, the success rates of immediate hemostasis of two devices were 100%, and there was no bleeding occurred within 24 h after surgery in patients. The operation time and closure intraoperative limb immobilization time were（6.0±1.21） min and（8±0.8） h using Perclose- Proglide vascular closure device, while they were（2.0 ±1.12） min and（8 ±0.5） h using Angio- seal（P〈0.05）. Conclusion Both vascular closure device can achieve the purposes of safe and effective hemostasis with no major vascular complications if we can master their characteristics and use skills and apply for the right patients.
