经皮氧分压检查对糖尿病下肢血管病变的诊断价值

摘　　要：目的探讨经皮氧分压（Transcutaneous oxygen pressure,Tc PO2）检查在糖尿病下肢血管病变中的诊断价值。方法对160例2型糖尿病患者进行经皮氧分压和下肢血管CT血管造影检查（CT angiography,CTA）,并进行比较,评估Tc PO2在诊断下肢血管病变中的敏感性、特异性、阳性预测值、阴性预测值,探讨Tc PO2检查的诊断价值。结果采用受试者工作特征曲线（Receiver operating characteristic curve,ROC）分析的方法,以下肢血管CTA检查作为金标准,Tc PO2检查的曲线下面积为0.88（95%CI：0.82~0.94）,取Tc PO2≤40作为切点,其敏感性为0.71,特异性为0.96,阳性预测值为0.91,阴性预测值为0.87。结论经皮氧分压检查在糖尿病患者下肢血管病变中具有较高的诊断价值,值得临床上应用和推广。Objective To evaluate the value of transcutaneous oxygen pressure（TcPO2） in diagnosis of peripheral arterial disease （PAD）in diabetic patients. Methods TcPO2 and CTA methods were performed on 160 diabetic patients with peripheral arterial disease.The sensibility, specificity, positive predictive value,negative predictive value of TcPO2 in diagnosis of PAD were measured by using receiver operating characteristic curve（ROC）. Results The area under ROC curve was 0.88 （95%CI： 0.82~0.94）CTA as the diagnosis standard. At the cut-off point of TcPOr--〈40,the sensitivity, specificity, positive predictive value and negative predictive value were 0.71,0.96,0.91 and 0.87, respectively. Conclusions TcPO2 is highly valuable for diagnosis of peripheral arterial disease in diabetic patients and it is worthy for clinical application.
