摘  要:目的:探讨彩超联合举宫器在阴道注水腹腔镜不孕症诊治中的作用与意义。方法:为143例术前考虑输卵管性不孕的患者行经阴道注水腹腔镜联合宫腔镜检查与诊治,将患者随机分为两组,72例采用彩超联合举宫器经阴道穿刺(研究组),71例直接经阴道后穹隆穿刺(对照组)。对比两组患者穿刺成功率、穿刺出血量、副损伤等指标。结果:研究组穿刺成功率为98.6%,穿刺出血量平均(1.1±0.2)ml,无副损伤发生。对照组穿刺成功率为87.3%,穿刺点出血量平均(10.3±0.3)ml,发生周围脏器损伤3例。结论:经阴道注水腹腔镜联合宫腔镜探查是治疗不孕症的微创术式,采用彩超联合举宫器辅助穿刺不仅增加了注水腹腔镜不孕症诊治的成功率,而且明显提高了穿刺安全性,值得推广应用。Objective: To investigate the role and clinical significance of color-ultrasonography and uterine manipulator in trans- vaginal hydro-laparoseopy (THL) for the diagnosis and treatment of infertility. Methods:One hundred and forty-three patients with tub- al infertility underwent THL and hysteroseopy for diagnosis and treatment. They were randomly divided into two group : study group ( n = 72 ) with transvaginal puncture guided by color-ultrasonography and uterine manipulator;control group (n = 71 ) with directly transvagi- nal puncture. Puncture achievement ratio, blood loss,operation impairment were compared and analyzed between two groups. Results:In study group, puncture achievement ratio was 98.6%, blood loss was ( 1.1 ± 0.2 ) ml, there was no operation impairment. In control group, puncture achievement ratio was 87.3%, blood loss was ( 10.3 ± 0.3 ) ml, there was 3 cases of surrounding organ impairment. Conclusions:THL and hysteroscopy is mini-invasive in diagnosis and treatment of infertility, color-uhrasonography and uterine manipu- lator guiding can not only increase achievement ratio of THL, but also increase the safety of puncture. It deserves to be recommended in the clinical application.
