摘　　要：目的皮肤良性病变患者的血清鳞状细胞癌抗原(squamous cell carcinoma antigen,SCCA)水平常出现明显升高的现象。文中探讨常见良性皮肤病患者的血清SCCA在皮肤良性病变中表达水平的差异。方法选取泸州医学院附属医院皮肤科2012年5月至2014年6月住院良性皮肤病患者497例,对14种常见皮肤病患者血清SCCA进行测定。结果116例银屑病和37例红皮病患者的SCCA阳性率均为90.5%和100%,SCCA阳性率>60%的常见皮肤病有湿疹(61.7%)、泛发性神经性皮炎(65.1%)、天疱疮(61.1%)、大疱性类天疱疮(62.2%)。与脓疱型、红皮型银屑病患者的Ln(SCCA值)比较[(2.86±1.46)ng/m L、(3.56±1.84)ng/m L],寻常型[(1.76±1.22)ng/m L]、关节型[(1.69±1.05)ng/m L]均降低,差异均有统计学意义(P<0.05)。结论血清SCCA值升高可能与常见皮肤病的皮损面积、角化异常和脱屑程度有关,可以作为皮肤病诊断和治疗效果评价的一项参考依据。Objective There is a significant increase of squamous cell carcinoma antigen（ SCCA） in patients with common skin diseases. The aim of this study was to investigate the difference expression of serum SCCA in patients with common skin diseases.Methods A total of 497 patients with 14 kinds of common skin diseases from the Department of Dermatology were recruited,and the serum SCCA was measured. Results The positive rates of SCCA in 116 patients with psoriasis and 37 patients with erythroderma were 90. 5% and 100%,respectively. The common skin diseases with positive rate of SCCA greater than 60% includes eczema（ 61. 7%）,generalized dermatitis（ 65. 1%）,pemphigus（ 61. 1%）,bullous pemphigoid（ 62. 2%）. The SCCA values in pustulosa（ 1. 76 ± 1. 22 ng/m L） and arthropathica psoriasis（ 1. 69 ± 1. 05 ng/m L） were decreased compared with pustular（ 2. 86 ± 1. 46 ng / m L） and arthropathica psoriasis（ 3. 56 ± 1. 84 ng / m L）（ P〈0. 05）. Conclusion The SCCA value may have a relationship with the increase of the performance of keratinization and desquamation in the skin diseases,and it could be used as a valuable indicator for the diagnosis and treatment in patients with cutaneous diseases.
