摘 要:目的研究早期应用益赛普对脓毒症大鼠心肌损伤及炎症因子表达的影响,并就其对心肌损伤机制的影响进行初步探讨。方法 108只雌性清洁级SD大鼠随机分为假手术组、脓毒症组与治疗组,假手术组行盲肠探查术,脓毒症组和治疗组两组行盲肠结扎穿孔术制作脓毒症模型,治疗组加用益赛普以干预,均在3、9、24、36、48、72 h时相点留取标本,每时相点6只大鼠。ELISA法检测大鼠血清心肌肌钙蛋白(cT nI)、肿瘤坏死因子-α(TNF-α)、白细胞介素-1(IL-1)浓度,Western blot法检测心肌核转录因子-κB p65(NF-κB p65)的表达,数据结果采用析因设计方差分析。结果 1脓毒症组与治疗组血清cT nI、TNF-α、IL-1、NF-κB p65水平均高于假手术组(P<0.05),两组浓度均于3 h达峰值,而后逐渐下降。治疗组每个时相点均低于脓毒症组;2血清TNF-α、IL-1浓度均与血清CTnI表达呈显著正相关性。结论脓毒症大鼠心肌早期发生损伤,应用益赛普能够降低血清TNF-α、IL-1的释放及心肌组织NF-κB p65的表达,减少机体炎症反应,起到保护心脏的作用。Objective To explore the early effect of etanercept on myocardial injury mechanism through the early study of influence of etanercept on myocardial injury and expression of inflammatory response in sepsis rats. Meth-ods 108 female SD rats were randomly divided into the sham operation group, the sepsis group and the treatment group. The sham operation group underwent exploratory surgery on cecum,while,the sepsis group and the treatment group were produced in accordance with the CLP to establish sepsis model. Besides, the treatment group was injec-ted with etanercept. Specimens of three groups were respectively taken at 3,9,24,36, 48 and 72 h time points. Each time point had six rats. The concentrations of serum cTnI, TNF-α, serum IL-1 were detected by ELISA meth-od. The expression of cardiac NF-κB p65 was detected by Western blot. Data were processed with analysis of vari-ance of factorial design. Results ① The concentrations of serum cTnI of sepsis group and treatment group peaked at 3 h, then progressively decreased,and they were higher than the concentrations of sham operation group(P 〈0. 05 ) . The concentrations of treatment group were lower than the concentrations of sepsis group at each time point;② The concentrations of serum TNF-α and IL-1 of sepsis group and treatment group peaked at 3 h, and then pro-gressively decreased,and they were higher than the concentrations of sham operation group(P〈0. 05). The concen-trations of treatment group were lower than the concentrations of sepsis group at each time point;③The concentra-tions of serum TNF-α and IL-1 were significantly positively correlated with the concentration of cTnI;④ The con-centrations of NF-κB p65 of sepsis group and treatment group peaked at 3 h, and then progressively decreased. They were higher than the concentrations of sham operation group(P〈0. 05). The concentrations of treatment group were lower than the concentrations of sepsis group at each time point. Conclusion Early myocardial damage occurs in septic rat
