腹部超声与阴道超声对异位妊娠的诊断价值对比

摘　　要：目的:对比腹部超声(trans-abdominal sonography,TAS)与阴道超声(trans-vaginal sonography,TVS)在异位妊娠诊断中的应用价值。方法:226例异位妊娠患者分别采用腹部超声及经阴道超声检查,对比两种方法阳性检出率,同时对比两种方法对不同类型异位妊娠的阳性检出率。结果:TAS对异位妊娠诊断阳性率为82.3%(186/226),显著低于TVS的94.7%(214/226)(P<0.01);TVS对相对稳定型异位妊娠的诊断阳性率为96.0%(48/50),显著高于TAS的64.3%(18/28)(P<0.01),而TAS对急诊型异位妊娠的诊断阳性率为95.4%(21/22),显著高于TVS的66.7%(10/15)(P<0.05)。结论:TVS检查具有分辨率高、距离靶器官近等多重优势,TAS适用范围广泛、操作灵活,在临床实践操作中应灵活结合两种检查方法,提高异位妊娠的超声诊断准确率。Objective： To contrast the diagnostic value of trans-abdominal sonography （ TAS ） and trans-vaginal sonography （ TVS） in the diagnosis of EP. Methods：226 patients with EP were detected with TAS and TVS,and then the positive detection rates were contrasted between the two methods. Results：The positive rate of TAS in the diagnosis was 82. 3%（186/226）,significantly lower than 94. 7%（214/226） in TVS（P〈0. 01）;The positive rate of TVS in the relative stability of EP was 96. 0%（48/50）,significantly higher than 64. 3%（18/28） in TAS（P〈0. 01）;The positive rate of TAS in the emergency EP was 95. 4%（21/22）,significantly high-er than 64. 3%（18/28） in TVS（P〈0. 01）. Conclusion：TVS has advantages such as high resolution,near distance to the target or-gans,TAS has wide application range,simple operation,and we should use the two methods in clinical practice to improve the accuracy of ultrasound diagnosis of EP.
