摘 要:目的检测2型糖尿病(T2DM)患者血清脂肪细胞因子chemerin表达水平,探讨其与糖尿病肾病(DN)的相关性。方法 T2DM患者78例,根据尿白蛋白/肌酐比率(UACR)分为单纯糖尿病组(UACR<30 g/mg,36例)和DN组(UACR≥30 g/mg,42例)。同期健康体检者30例作为对照组。采用酶联免疫法检测血清chemerin表达水平。对3组生化指标、chemerin水平等进行比较,并分析它们之间的相关性。结果 DN组血清chemerin水平(149.90±25.71)μg/L显著高于单纯糖尿病组(123.11±22.04)μg/L(P<0.01);单纯糖尿病组与对照组(121.03±21.89)μg/L比较血清chemerin无明显差异(P>0.05);T2DM患者血清chemerin水平与收缩压(SBP)、体重指数(BMI)、胰岛素抵抗指数(HOMA-IR)、血肌酐(Scr)和甘油三酯(TG)呈正相关(均P<0.05),与肌酐清除率(Ccr)、高密度脂蛋白(HDL-C)和血脂联素呈负相关(均P<0.05);多元回归分析显示Ccr、UACR是影响T2DM患者血清chemerin主要因素。结论血清chemerin水平升高与DN相关,可作为预测DN的参考标记物。Objective Tostudythe changes of serum chemerin levels and investigate the relationship between serum chemerin and diabetic nephropathy( DN) in patients with type 2 diabetes mellitus( T2DM). Methods According to the urinary albumin creatinine ratio( UACR),78 T2 DM patients were divided into the pure T2 DM group( 36 cases with UACR ﹤30 g / mg) and the DN group( 42 cases with UACR≥30 g / mg). 30 healthy people for regular checkup were selected as the control group. Serum chemerin levels were measured by ELISA. The biochemical indicators and serum chemerin levels were compared in three groups and the relationship between the serum chemerin levels and the biochemical indicators was analyzed. Results Chemerin level was significantly higher in the DN group[( 149. 90 ± 25. 71) μg / L] than that in the pure T2 DM group [( 123. 11 ± 22. 04) μg / L]( P〈0. 01). There was no significant difference between the pure T2 DM group and the control group in serum chemerin level. Pearson correlation analysis showed that the serum chemerin level was positively correlated with systolic pressure( SBP),body mass index( BMI),homeostasis model assessment of insulin resistance( HOMA- IR),Scr,triglyceride( TG)( all P〈0. 05) and negatively correlated with serum adiponectin,high- density lipoprotein cholesterol( HDL- C),and creatinine clearance rate( Ccr)( all P〈0. 05). Multiple regression analysis showed that the Ccr and UACR were the major factors which influenced serum chemerin. Conclusion Serum chemerin can be taken as a predictive marker in DN.
