摘　　要：目的 探讨N-端脑钠肽和左心房容积指数评价心房颤动（房颤）患者左心室舒张功能的价值。方法 行首次射频消融治疗的孤立性房颤患者47例,根据左心室舒张功能情况分为左心室功能正常组37例（E/e〈10）和左心室功能不全组10例（E/e〉10）,比较2组血清N-端脑钠肽（N-terminal B-type natriuretic peptide,NT-pro BNP）水平和心脏超声指标。结果 左心室舒张功能不全组血浆BNP水平[94（78.7,223.71）ng/L]、左心房容积指数[（39.9±11.6）mL/m2]明显高于左心室舒张功能正常组[54.8（39.7,89.9）ng/L、（32.1±7.4）mL/m2]（P〈0.05）;BNP对数、左心房容积指数及2个指标联合判断房颤患者左心室舒张功能不全的AUC分别为0.801（95%CI：0.653~0.949,P=0.009）、0.741（95%CI：0.535~0.947,P=0.036）、0.810（95%CI：0.658~0.963,P=0.007）。结论 BNP和左心房容积指数联合检测可作为早期房颤患者左心室舒张功能不全的诊断指标之一。Objective To evaluate the value of N-terminal B-type natriuretic peptide （BNP） and left atrial volume index （LAVI） to the evaluation of left ventricular diastolic function （LVDF） in atrial fibrillation patients. Methods Forty seven patients receiving initial catheter radiofrequency ablation for lone arterial fibrillation were divided into normal LVDF group （n=37） （E/e＇〈10） and abnormal LVDF group （n：10） （E/e＇〉10）. BNP levels and cardiac ultrasound indexes were compared between two groups. Results BNP level and LAVI were significantly higher in abnormal LVDF group （94（78.7, 223.7） ng/L, （39.9±11.6） mL/m2） than those in normal LVDFgroup （54.8（39.7, 89.9） ng/L, （32. 1± 7.4） mL/m2） （P〈0.05）. AUC values of BNP, LAVI and BNP plus LAVI for left ventricular diastolic dysfunction were 0.801 （95% CI： 0.653 to0.949, P=0.009）, 0.741 （95MCI： 0.535 to 0.947, P=0.036） and 0.810 （95% CI： 0.658 to 0. 963, P=0. 007）. Conclusion The combination detection of BNP and LAVI may be used as one of the diagnosis indexes of left ventricular diastolic dysfunction in patients with atrial fibrillation.
