CT引导下肺组织穿刺活检病理检查联合组织培养诊断特殊类型肺炎的价值

摘　　要：目的探讨CT引导下肺组织穿刺活检病理检查联合组织培养在特殊类型肺炎诊断中价值。方法回顾性分析23例特殊类型肺炎患者临床资料,分析CT引导下经皮肺穿刺活检术检出标本组织病理和组织培养、涂片检查结果。结果 23例穿刺均取得肺组织,穿刺成功率为100%;组织活检病理诊断特殊类型肺炎17例,组织培养诊断15例,二者联合诊断21例;与临床诊断结果进行比较,联合诊断符合率为91.3%(21/23),2例因坏死物较多未能明确诊断;穿刺后5例少量气胸,3例少量出血经对症处理后症状消失。结论 CT引导下经皮肺穿刺组织活检病理检查联合组织培养诊断特殊类型肺炎准确性高,且安全。Objective To evaluate value of CT-guided pereutaneous core needle biopsy （PCNB） and tissue culture to the diagnosis of special pneumonia. Methods The clinical data of 23 cases of special type of pneumonia were retrospectively studied, and the histopathologic results of PCNB were analyzed. Results All 23 patients obtained needle biopsy specimen, with the success rate of 100~. Special type of pneumonia was confirmed by histopathologic examination in 17 cases, by further tissue culture in 15 cases, and by PCNB plus tissue culture in 21 cases. The coincidence rate of PCNB plus tissue culture was 91.3% （21/23）. Two cases were unable make diagnosis due to more necrosis. In 5 cases of sligh bleeding after PCNB, bleeding was stopped in 3 cases after treatment. Conclusion PCNB plus tissue culture is a safe technique with high accuracy for the diagnosis of special type of pneumonia.
