摘  要:目的分析维持性血液透析(maintenance hemodialysis,MHD)患者血清硫化氢(hydrogen sulfide,H2S)水平及颈动脉内中膜厚度(intima-media thickness,IMT)变化及其相关性。方法 87例MHD患者为MHD组,同期体检健康者40例为对照组,比较2组血清H2S、颈动脉IMT,分析MHD患者血清H2S与颈动脉IMT的相关性。结果 MHD组血清H2S[(6.6±3.2)nmol/mL]低于对照组[(14.9±7.6)nmol/mL](P<0.01),颈动脉IMT[(1.9±1.0)mm]高于对照组[(0.7±0.6)mm](P<0.01);校正传统心血管混杂因素、估计肾小球滤过率及透析龄后,MHD患者血清H2S(r=-0.281,P=0.000)、年龄(r=0.279,P=0.000)、钙磷乘积(r=0.215,P=0.000)、全段甲状旁腺激素(r=0.202,P=0.000)与IMT相关。结论血清H2S是导致MHD患者IMT发生改变的独立危险因素。Objective To explore the relationship between serum hydrogen sulfide (H2 S) and carotid artery intima-media thickness (IMT) in patients with maintenance hemodialysis (MHD). Methods The serum H2S and carotid IMT score were detected in 87 MHD patients (MHD group) and 40 healthy controls (control group), and the relationship between H2S and IMT was analyzed. Results H2S level was lower in MHD group ((6.6±3.2) nmol/mL) than that in control group ((14.9±7.6) nmol/mL) (P〈0.01), and IMT was larger in MHD group ((1.9±1.0)mm) than that in control group ((0.7 ± 0.6) mm) (P(0.01). After correcting cardiovascular confounding factors, estimated glomerular filtration rate and hemodialysis duration, the serum H2S level (r=0. 281, P=0. 000), age (r=0. 279, P=0. 000), calcium phosphate product level (r=0. 215, P=0.000) and pare thyrosdism hormone level (r=0. 202, P=0.000) were correlated with IMT. Conclusion The serum H2 S is an independent risk factor for IMT changes in MHD patients.
