老年慢性心力衰竭的病因、心衰类型及心功能的临床分析

摘　　要：目的：回顾性分析224例老年慢性心衰患者的病因构成、心衰类型及心功能特点。方法全部患者以年龄60～69岁、70～79岁、≥80岁分为3组，比较分析不同年龄阶段患者病因、伴随疾病、心衰类型（收缩功能减退、舒张功能减退、混合性减退）及心功能（ NYHA分级）情况，收集心衰指标（多次住院患者以最后一次为准）E/A、LVEF（%）、LAD（ mm）、LVDd（ mm）。结果老年慢性心衰患者在基础疾病、伴随疾病、心衰类型、心功能评级、超声心动图及多病因组成等方面，三组间差异有显著意义。结论老年慢性心衰患者主要以冠心病、高血压、肺心病等为主要基础疾病，以慢性阻塞性肺疾病、心律失常、糖尿病等为主要伴随疾病；心衰类型主要以舒张性心衰为主，心功能以Ⅲ级为主；老年慢性心衰患者存在明显的多病因性，在治疗上应进行多元化及个体化防治。Objective：To retrospectively analyze the etiology,category and cardiac function of 224 elderly patients with chronic heart failure. Methods：All patients were divided into three groups：60-69,70-79,and above 80 years old. The different age-stages of etiology,concomitant diseases and category of heart failure（ systolic heart failure,diastolic heart failure,mixed heart failure）and cardiac function（ NYHA classification）were analyzed. And the heart failure marks（ hos-pitalized patients will be subject to the last time many times）such as E/A,LVEF（%）,LAD（ mm）and LVDd（ mm）were collectd. Results：Elderly patients with chronic heart failure in the basic diseases,concomitant diseases,category of heart failure,cardiac function rating,echocardiography and constituent features of the multifactor,had a significant difference between the three groups. Conclusion：In elderly patients with chronic heart failure the predominant basic diseases are coro-nary heart disease（CHD）,pulmonary heart disease（PHD）and hypertension etc. And their concomitant diseases mainly are chronic obstructive pulmonary disease（ COPD）,cardiac arrhythmia and diabetes. Heart failure category is mainly com-posed of diastolic heart failure,and cardiac function is mostly NYHA Ⅲ. Most of the elderly patients with chronic heart failure are caused by multiple etiological factors,who should be given multiple and characterized prevention and treatment according to the multiple factors .
