高级别星形细胞瘤与良性脑膜瘤瘤周水肿区磁共振扩散加权成像表观扩散系数值评价

摘　　要：目的利用表观扩散系数(ADC)值探讨高级别星形细胞瘤与良性脑膜瘤瘤周水肿区性质。方法 50例患者术前行磁共振磁敏感加权成像(SWI)、磁共振扩散成像(DWI),绘制ADC图,测量瘤周区、远离肿瘤区、对侧相应部位正常脑白质ADC值。2组ADC值组间区别进行t检验。结果瘤内磁敏信号(ITSS)3级星形细胞瘤组瘤周ADC值为(1.25±0.18)×10-3 mm2/s,低于ITSS1、2级组ADC值(1.66±0.15)×10-3 mm2/s,两者之间差异有统计学意义;ITSS 1、2级组ADC值低于良性脑膜瘤组ADC值(1.76±0.15)×10-3 mm2/s,两者之间差异无统计学意义。结论 ITSS 3级星形细胞瘤组瘤周血管源性水肿与细胞毒性水肿相互作用,导致ADC值降低。ITSS 1、2级星形细胞瘤与良性脑膜瘤瘤周水肿主要为血管源性水肿。Objective Using the apparent diffusion coefficient（ADC）value of high grade astrocytoma and benign meningioma peritumoral edema region of peritumoral edema region properties. Methods Fifty patients underwent susceptibility weighted imaging（SWI）,diffusion weighted imaging（DWI）,drawing ADC,measurement of peritumoral areas,away from the tumor area,contralateral normal white matter ADC value position. The 2 group of ADC value between group difference by t test. Results Grade（intratumoral susceptibility signal intensity（ITSS） 3 astrocytoma peritumoral ADC value of（1.25 ±0.18）×10-3mm2/s,less than ITSS 1,2 level set of ADC values is（1.66 ±0.15）×10^-3mm^2/s,with statistically significant difference;ITSS 1,level 2 group ADC value is lower than the ADC value of benign meningioma group is（1.76±0.15）×10^-3mm^2/s,no difference statistically between the two significance. Conclusion In the peritumoral edema region of graded ITSS 3 astrocytomas,with the interaction of vasogenic edema and cytotoxic edema,the ADC value decreased.
