摘 要:目的探讨首发未治疗重性抑郁症(MDD)患者经针刺百会穴(GV20)后脑功能区的低频振幅(ALFF)变化。方法将16例首发未治疗MDD患者依据针刺真假情况,分别纳为GV20组和假穴组,另将20名健康自愿者纳为对照组。于针刺真、假穴前后分别行静息态f MRI扫描,应用ALFF方法分析受试者脑功能区变化情况。并采用统计学方法,检验GV20组与另两组间ALFF变化的差异。结果针刺前,GV20组ALFF值较对照组升高的区域主要位于右侧前、中扣带回、豆状核、楔前叶及两侧岛叶,降低的区域主要位于两侧海马、小脑及枕叶;针刺真、假穴后,与假穴组对比,GV20组右侧前、中扣带回、豆状核及左侧岛叶ALFF值降低,左侧海马、小脑ALFF值升高,与针刺前相应异常脑区的变化相反。结论针刺GV20可能对MDD患者部分异常的脑功能区有调节作用。基于f MRI技术的ALFF算法可为探索针刺治疗MDD的中枢机制提供潜在的影像学证据。Objective To investigate the changed amplitude of low frequency fluctuation( ALFF) of functional brain regions in on site,untreatmented patients with major depressive disorder( MDD) after electric acupuncture stimulation( EAS) at Baihui( GV20). Methods Sixteen on site,untreatemented patients with MDD were divided into GV20 group and sham group based on EAS at GV20 or sham acupoint,and twenty cases of healthy volunteers were delimited as control group. The three group enrolled in rest state f MRI scanning before and after EAS at GV20 and the sham acupoint separately. The differences of ALFF between GV20 group and the other two groups were detected by statistical methods. Results Before EAS,compared to healthy subjects,increased ALFF regions of GV20 group were mainly located in the right anterior cingulate cortex,middle cingulate cortex,putamen,precuneus,bilateral insula,and decreased ALFF regions mainly were located in the bilateral hippocampus,cerebellum and occipital lobe; After EAS at GV20 and sham acupoint,compared to the sham group,decreased ALFF regions of GV20 group were mainly located in the right anterior cingulate cortex,middle cingulate cortex,putamen,the left insula,and increased ALFF regions were mainly located in the left hippocampus and cerebellum,and which were the main regions showing differences between GV20 group and control group before EAS.Conclusion Some brain areas of MDD patients might been modulated after EAS at GV20. The ALFF method based on f MRI might provide a potential imaging evidence for studying neural mechanisms of EAS treating patients with MDD.
