摘  要:目的探讨应用Care Dose 4D及Care k V技术降低下肢深静脉及肺动脉CT血管成像辐射剂量的可行性。方法搜集因下肢深静脉血栓或肺动脉栓塞在行下肢深静脉CT血管成像检查者200例,全部患者均在双源CT上完成双下肢至肺动脉全程血管的检查。图像后处理在Siemens Syngo via工作站完成,包括最大密度投影、多平面重组和容积再现法重建,判断并记录下肢深静脉血栓、肺栓塞、血管变异以及相关血管病变情况,其中A组(100例)患者未采用Care Dose 4D及Care k V技术与B组(100例)患者使用该技术,比较A组与B组辐射剂量之间的差异。结果下肢深静脉CTV检查显示肺动脉栓塞43例,其中伴发深静脉血栓39例,下肢深静脉血栓87例,Cockett综合征20例,布加综合征6例,血管变异与侧支循环开放35例,静脉受压9例。B组平均有效辐射剂量[(491.5±156)m Gy/cm]较A组平均有效辐射剂量[(1164±451)m Gy/cm]显著降低(P<0.01)。结论采用自动辐射剂量调整技术(Care Dose 4D和Care k V)行双下肢深静脉及肺动脉血管成像可在保证清晰显示下肢深静脉血栓、肺动脉栓塞及伴发侧支循环的同时,显著降低辐射剂量。Objective To investigate feasibility of CARE k V Care Dose 4D technology to reduce deep venous and pulmonary CT angiography radiation dose. Methods From January 2012 to August 2014,200 cases underwent deep venous CT angiography because of lower extremity deep venous thrombosis( DVT) or pulmonary embolism( PE) in our hospital.All patients were examined by dual- source CT both the entire lower extremity and the pulmonary vascular examination.Post process of imagings had been completed in the Siemens Singo workstations,including maximum intensity projection( MIP),multiplanar reconstruction( MPR) and volume rendering( VR) reconstruction. Deep vein thrombosis,pulmonary embolism,abnormal blood vessels,and related vascular lesions had been analyzed and recorded. Group A( n = 100) were not used Care k V Care Dose 4D technology( control group) and group B( 100 patients) in patients using this technology,radiation dose had were compared between group A and group B. Results CTV examination revealed deep venous pulmonary embolism( n = 43),which associated with 39 cases of deep vein thrombosis( 90. 6%),87 cases of deep vein thrombosis,20 cases with cockett syndrome,Budd- Chiari syndrome in 6 cases,abnormal blood vessels and collateral circulation open in 35 cases,9 cases of vein compression. The mean effective radiation dose( 491. 5 Soil 156) m Gy / cm compared between A and B group,the average effective radiation dose( 1164 Soil 451) m Gy / cm was significantly lower( P〈0. 01).Conclusion The use of automatic radiation dose adjustment technology( Care Dose 4D and Care k V) double row deep venous and pulmonary angiography could display DVT clearly,PE and concomitant collateral circulation at the same time,significantly reduce the radiation dose.
