关节镜下同种异体半月板移植术后疗效观察以及关节镜二次探查报道  

Clinical Outcome of Arthroscopic Meniscal Allograft Transplantation and the Follow-up Results of Second-look Arthroscopy after the Transplantation

摘　　要：目的：探讨18例关节镜下同种异体半月板移植术后效果,并且对其中6例进行术后二次探查以进一步分析移植术后临床效果。方法：对18例2010年1月至2013月12月间半月板切除术后的患者,在关节镜辅助下实施同种异体半月板移植术,对患者进行了平均18.80±0.51（15-26）个月的随访,采用IKDC、Lysholm和Tegner评分进行评估比较,观察分析其恢复效果。其中6例患者术后行关节镜二次探查。结果：18例半月板移植患者行走功能正常,膝关节活动度良好,术后膝关节疼痛不适明显改善,可以完成日常活动和基本体育运动,各项评估指标如IKDC、Lysholm、Tegner评分均较术前有明显提高。其中6例关节镜二次探查见移植半月板周缘关节囊愈合,形态基本正常,稳定性良好。结论：关节镜下同种异体半月板移植安全,半月板移植可有效缓解半月板切除后出现的关节疼痛、肿胀等症状,可获得较好的近期临床效果,临床未见明显的免疫排斥反应。Objective To investigate the clinical outcome of arthroscopic meniscal allograft transplantation,and the results of arthroscopic second look after the transplantation. Methods From January 2010 December to2013 February,arthroscopic knee meniscal allograft transplantation after meniscectomy were performed in 18 cases in our hospital. The follow-up duration was of 18.80 ± 0.51（15 -26） months and the IKDC,Lysholm and Tegner scores were evaluated at the same time. 6 of the 18 cases received second-look arthroscopy after the transplantation. Results All patients walked normally after the transplantation with significant improvement of knee pain and knee discomfort,range of movement,KDC,Lysholm,and Tegner scores. Complete healing of the joint capsule accompanied with normal morphology and stability of knee joint were observed during the second-look arthroscopy. Conclusion Arthroscopic meniscal allograft transplantation effectively relieves joint pain,swelling and other symptoms after meniscectomy,and obtains satisfactory clinical results with no significant immune rejection.
