DSCT双能量成像碘图在颈部肿大淋巴结中的应用

摘　　要：目的探讨双源CT双能量成像碘图在颈部不同病理性质肿大淋巴结诊断中的应用价值.方法回顾性分析94枚经病理证实的颈部肿大淋巴结双能量模式增强扫描CT资料,利用"Liver VNC"软件处理得到碘图.对不同病理性质肿大淋巴结增强双期所得碘图的平均CT值进行比较,观察不同病理性质肿大淋巴结的强化特点.结果动脉期不同病理性质肿大淋巴结间碘图的平均CT值有差异(F=8.558,P<0.001);实质期不同病理性质肿大淋巴结间碘图的平均CT值有差异(F=8.558,P<0.001).结论增强双期双能量扫描模式所得碘图的平均CT值在不同病理性质肿大淋巴结间有差异,比较增强双期碘图的平均CT值对淋巴结的定性诊断有一定的临床实用价值.Objectives To investigate the clinical value of dual source CT（DSCT） dual energy（DE）technique in the cervical lymph nodes.Methods The contrast images in two phases of 94 cervical lymph nodes were retrospectively analyzed. The degree of enhancement of enlarged lymph nodes in arterial and parenchymal phases on iodine- enhanced and weighted fusion 120 k V images were evaluated. The mean CT values of benign and malignant lymph nodes in the two phases on iodine- enhanced and weighted fusion 120 k V images for were measured to evaluate the effect of mean CT values for differentiating the benign and malignant cervical lymph nodes. The mean CT values of dual- phase enhancemen on iodine- enhanced image for the enlarged lymph nodes derived from different pathological change were compared to conclude the enhanced characteristics for the different pathological enlarge lymph nodes.Results（ 1） The mean CT values of benign and malignant enlarged lymph nodes on iodine- enhanced images in arterial and parenchymal phase was no difference between them（P〈0.05）.2.There were statistical differces in CTvalues among different pathological types in arterial and parenchymal phase（F=8.558, P〈0.001）.Conclusions The mean CT value of different pathological types of lymph nodes on iodine- enhanced image has statistically significant difference on dual- phase mode.Measuring the mean CT value of dual- phase iodine- enhanced images was helpful to make qualitative diagnosis of lymph nodes lesions.
