血浆超敏C反应蛋白和同型半胱氨酸水平与2型糖尿病周围神经病变的相关性研究

摘　　要：目的探讨分析血浆超敏C反应蛋白(hs-CRP)及同型半胱氨酸(Hcy)与2型糖尿病周围神经病变(DPN)的相关性。方法将120例2型糖尿病住院患者,按神经自主症状和肌电图检查分为单纯糖尿病患者组(SDM组)和糖尿病周围神经病变组(DPN组),所有患者测血浆hs-CRP、Hcy水平、空腹血糖和糖化血红蛋白(Hb A1C),同时测定血浆甘油三酯(TG)、总胆固醇(TC),低密度脂蛋白胆固醇(LDL-C)和高密度脂蛋白胆固醇(HDL-C)水平。结果糖尿病周围神经病变组患者hs-CRP和Hcy水平明显高于单纯糖尿病组hs-CRP和Hcy水平,差异有统计学意义(P<0.05)。Pearson相关性分析结果显示:糖尿病神经病变患者血清hs-CRP水平与Hcy水平呈正相关。Logistic回归分析显示:糖尿病病程、糖化血红蛋白、血浆hs-CRP和Hcy水平与DPN相关,是发生DPN的危险因素。结论血浆hs-CRP和Hcy与糖尿病周围神经病变发生密切相关,血浆hs-CRP和Hcy水平检测在糖尿病周围神经病变患者诊断与治疗中有较好的临床应用价值。[ Objective ] To study the correlation of the plasma high sensitivity C-reactive protein （hs-CRP） and homoeysteine （Hcy） with peripheral neuropathy in type 2 diabetic patients. [ Methods ] One hundred and twenty diabetes mellitus （DM） patients were divided into simple DM （SDM） group and diabetic peripheral neuropathy （DPN） group according to their subjective symptoms and electromyography. The plasma hs-CRP and Hcy, glucose, HbA1C, the plasma TG, TC, LDL-C, HDL-C were measured. [ Results ] The level of plasma hs-CRP and Hey in SDM group was significantly higher than that in the control group （/9 〈 0.05）, while in DPN group, it was significantly higher than in the SDM group （P 〈 0.05）. The Pearson correlation analysis showed that the level of hs-CRP was positively correlated with Hcy. Logistic regression analysis showed that the course of DM, HbA1C, the level of plasma hs-CRP and Hey were correlated with DPN, and they were risk factors for DPN. [ Conclusions ] It is suggested that the level of plasma hs-CRP and Hey would be closely related with diabetic peripheral neuropathy and it could be a useful diagnostic marker for the peripheral neuropathy in diabetic patients.
