老年慢性肾脏疾病并发急性肾损伤的临床特点及危险因素分析

摘　　要：目的探讨老年慢性肾脏疾病并发急性肾损伤的临床特点及危险因素。方法通过对2009年1月至2014年12月在我院进行治疗的74例慢性肾脏疾病并发急性肾损伤患者的临床资料进行回顾性分析,根据患者年龄分为老年组和中青年组。观察比较两组患者疾病构成比、危险因素及治疗效果差异。结果老年慢性肾脏疾病并发急性肾损伤的疾病构成以糖尿病肾病、慢性肾小球肾炎和高血压肾损害为主,危险因素主要为低血容量、严重感染及肾毒性药物,老年患者治疗总有效率同中青年组比较明显下降,差异具有统计学意义(P<0.05)。结论多种因素可诱发老年慢性肾脏疾病并发急性肾损伤,在临床治疗过程中应重视高危因素,及时发现并采取积极控制措施,针对原发疾病应用整体治疗,可有效避免急性肾损伤。Objective To investigate the clinical characteristics of chronic kidney disease complicated by acute kidney injury of elderly patients and its risk factors. Methods Retrospectively analyzed the clinical datas of74 cases of patients those who suffered chronic kidney disease complicated acute kidney injury and hospitalized in our hospital during January 2009 and December 2014. They were divided into old age group and the young group according to the patient＇s age. The differences of constituent ratio of disease,risk factors and treatment efficacy between the two groups were compared. Results Elderly patients with chronic kidney disease complicated by acute kidney injury diseases mainly including diabetic osteopathy,chronic conglomeration and hypertensive renal damage,the main risk factors were hypoglycemia,severe infection and renal toxicity induced by drugs. Efficacy of elderly patients decreased compared with the young group,the difference was statistically significant（ P〈0.05）. Conclusions Many factors can induce acute kidney injury of elderly patients with chronic kidney disease,the clinical course of treatment should pay attention to high risk factors. To discover and to take active control measures for the applications and treatment of primary disease can effectively prevent acute kidney injury.
