摘 要:目的了解奥沙利铂并大剂量X线盆腔照射对正常大鼠直肠组织的损伤。方法将120只Wistar大鼠随机分成A、B、C、D组,D组给予腹腔注射生理盐水及假照,A组腹腔注射奥沙利铂15 mg/kg,B组盆腔给予6 MV X线12Gy单次大剂量照射;C组腹腔注射奥沙利铂15mg/kg联合6 MV X线12Gy单次大剂量照射。照射后第3、7、14、21、42天,各组分别处死6只大鼠,观察直肠组织的形态学改变,免疫组织化学法检测Bax和Bcl-2蛋白表达情况。结果 C组大鼠照射后第7天直肠杯状细胞大量减少,固有层腺体萎缩,黏膜下可见大量的炎细胞浸润及纤维化形成;A、B组出现类似症状,程度较轻。A、B、C组各时间点的Bax表达阳性率比较差异均有显著性,其中第14、21、42天Bax表达阳性率较第3、7天降低,差异有显著性(F=129.1~550.8,P〈0.01)。A、B、C组照射后的第3、7天Bax表达阳性率较D组明显升高,其中C组Bax表达阳性率均较同期的A、B组高,差异有显著性(F=596.9、134.8,P〈0.01)。A、B、C组各时间点的Bcl-2表达阳性率比较差异均有显著性,3组第7天时表达率低于其他时间点(F=199.3~548.2,P〈0.01);C组Bcl-2表达阳性率较同期的A、B、D组低(F=25.9,P〈0.01)。随着时间延长,C组Bcl-2表达阳性率呈一定的上升趋势,第14、21、42天均较第7天升高,差异有显著性(F=548.2,P〈0.01)。结论同期放化疗对直肠组织损伤较单纯化疗、单纯放疗严重,后期出现代偿性修复的机制可能与Bax、Bcl-2表达的变化有关。Objective To investigate the damage of normal rectum in rats caused by oxaliplatin combined with a single large-dose of radiation. Methods A total of 120 Wistar rats were equally randomized to four groups as groups A,B,C and D.The rats in group D were intraperitoneal injection of normal saline and pseudo radiation;those in group A were intraperitoneal injection of oxaliplation(15mg/kg);those in group B were given 6MV X-ray at single dose of 12 Gy irradiating on pelvic cavity;and those in group C received 6 MV X-ray at a single dose of 12 Gy irradiating pelvic cavity after intraperitoneal injection of 15mg/kg oxaliplation.Six rats in each group were killed in each time on days 3,7,14,21 and 42after radiation.Morphological changes were observed by HE staining.Expressions of Bcl-2and Bax were examined using immunohistochemical technique. Results On day 7after radiation,the goblet cells in rectum of rats in group C markedly decreased,glands in lamina propria atrophied,and a large quantity of submucosal inflammatory cellular infiltration and fibrosis could be seen,similar reactions were also observed in groups A and B,but slighter.Expressions of Bax were significantly different between each time point of groups A,B and C,of which,the expressions on days 14,21 and 42were lower than that on days 3and 7(F=129.1-550.8,P〈0.01).On days 3and 7after radiation,the expression of Bax in groups A,B and C was elevated compared with group D,of which,the expression of Bax in group C was higher than that in groups A and B of corresponding time(F=596.9,134.8;P〈0.01).The difference of expression of Bcl-2in groups A,B and C between different time points was significant,the expression rate of the three groups on day 7was lower than the other time points(F=199.3-548.2,P〈0.01).The positive expression rate of Bcl-2in group C was lower than groups A,B and D of the same time period(F=25.9,P〈0.01).Along with the extension of time,there was a tendency of unvarying elevation of Bcl-2expression,being higher on days 14,
