^(131)Ⅰ全身显像与CT及X线胸片对分化型甲状腺癌肺转移诊断价值

摘　　要：目的探讨分化型甲状腺癌(DTC)肺转移清除转移灶(清灶)治疗后131Ⅰ全身显像(131Ⅰ-WBI)、胸部CT与X线胸片对DTC肺转移灶的诊断价值。方法回顾分析49例DTC肺转移病人131I清灶治疗后的131Ⅰ-WBI、胸部CT与X线胸片的影像学表现特点,比较三者对DTC肺转移的诊断灵敏度。结果清灶后131I-WBI、胸部CT、X线胸片诊断DTC肺转移的灵敏度分别为83.7%、79.6%、39.1%,清灶后131I-WBI、胸部CT诊断DTC肺转移的灵敏度与X线胸片比较,差异有显著性(χ2=17.0、15.3,P<0.05),而131I-WBI与CT间比较差异无显著性(P>0.05)。结论 131Ⅰ-WBI与胸部CT对于DTC肺转移灶的诊断灵敏度高于X线胸片。Objective To assess the diagnostic value of ^131I-WBI,CT and chest X-ray for patients with differentiated thyroid cancer（DTC）after removing the lung metastases. Methods A retrospective study was done in 49 DTC patients with pulmonary metastases after resection of the metastatic lesions.The imaging characteristics of ^131I-WBI,chest CT and chest X-ray were observed,and the sensitivity between the three methods in the diagnosis of DTC with lung metastases was compared. Results The sensitivity of ^131I-WBI,CT and chest X-ray after removal of metastases was 83.7%,79.6%,and 39.1%,respectively,the difference of that between both ^131I-WBI,CT and chest X-ray was significant（χ^2=17.0,15.3;P〈0.0167）,and no significant difference was seen between ^131I-WBI and CT（P〉0.05）. Conclusion The sensitivity of ^131I-WBI and CT is higher than chest X-ray in the diagnosis of lung metastasis caused by differentiated thyroid cancer.
