红参五子丸治疗原发性甲状腺功能减退症脾肾阳虚型40例临床观察

摘　　要：目的观察红参五子丸治疗原发性甲状腺机能减退症脾肾阳虚型的临床疗效及安全性。方法收集原发性甲状腺机能减退症脾肾阳虚型患者80例,随机分为治疗组和对照组各40例。治疗组口服红参五子丸每次10 g,每日3次,加左甲状腺素钠片模拟剂每次50μg,每日1次,对照组口服左甲状腺素钠片加红参五子丸模拟剂,用法用量同治疗组。两组均连续治疗3个月。观察治疗前后两组患者主要中医症状评分,检测血清游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)、促甲状腺素(TSH)水平,比较疾病疗效、中医证候疗效及安全性。结果治疗组疾病疗效总有效率为89.74%,对照组为86.84%,两组比较差异无统计学意义(P>0.05);治疗组中医证候疗效总有效率为97.44%,明显高于对照组的81.58%(P<0.01)。两组治疗后主要中医症状各单项症状评分及总积分均较本组治疗前明显降低,并且治疗组低于对照组(P<0.05)。治疗后两组血清FT3、FT4较本组治疗前均有升高,TSH下降(P<0.05),两组间比较差异无统计学意义(P>0.05)。治疗期间两组均未发生不良反应。结论红参五子丸治疗原发性甲状腺机能减退症脾肾阳虚型疗效肯定且安全性好,并在改善症状方面优于左甲状腺素钠片。Objective To observe the clinical effect and safety of Hongshen Wuzi Wan（ 红参五子丸） in treating primary hypothyroidism with spleen and kidney Yang deficiency. Methods Totally 80 patients with primary hypothyroidism with spleen and kidney Yang deficiency were randomly divided into a treatment group and a control group each group with 40 cases. Treatment group was given 10 g of Hongshen Wuzi Wan orally,three times a day,and 50μg of placebo of levothyroxine sodium tablets once a day. Control group was given equivalent levothyroxine sodium tablets and placebo of Hongshen Wuzi Wan. The treatments lasted for 3 months for both groups. Chinese medicine symptom scores were observed,levels of free triiodothyronine（ FT3）,free thyroxin（ FT4） and thyroid stimulating hormone（ TSH） were detected,and the improvement of disease,Chinese medicine syndromes and safety were compared before and after treatment. Results The total improvement rate of disease was 89. 74% in treatment group,and 86. 84%in control group,with no significant difference between the groups（ P〉0. 05）. The total improvement rate of Chinese medicine syndrome in treatment group was 97. 44%,which was much higher than 81. 58% in control group（ P〈0. 01）. The scores of single symptom and total Chinese medicine scores in both groups were decreased after treatment,especially in the treatment group（ P〈0. 05）. Levels of FT3 and FT4 were increased and those of TSH were decreased in both groups,with no significant difference between the groups（ P〉0. 05）. Conclusion Hongshen Wuzi Wan is effective and safe in treating primary hypothyroidism with spleen and kidney Yang deficiency,and is superior to levothyroxine sodium tablets in improving symptoms of the disease.
