摘 要:目的探讨血清胱抑素C(Cys-C)及血、尿β2微球蛋白(β2-MG)与新生儿窒息性肾损伤的关系及预后价值。方法将2013年7月至2014年6月广西柳州市柳铁中心医院儿科住院的68例窒息性肾损伤新生儿,按窒息程度分为轻度窒息组32例、重度窒息组36例,给予常规对症治疗。随机选择健康新生儿30例为对照组,分别于出生后24h内及出生3天后检测3组新生儿血Cys-C、血β2-MG、尿β2-MG、血尿素氮(BUN)及血肌酐(CR)水平。结果轻度及重度窒息组血Cys-C及血、尿β2-MG水平均较对照组显著升高(轻度窒息组与对照组比较,t值分别为7.213、6.529、9.006;重度窒息组与对照组比较,t值分别为9.586、9.087、14.189;均P<0.01);重度窒息组BUN、CR水平较对照组及轻度窒息组均显著升高(重度窒息组与对照组比较,t值分别为8.956、7.323;重度窒息组与轻度窒息组比较,t值分别为7.634、6.373;均P<0.01);轻度及重度窒息组治疗后血Cys-C及血、尿β2-MG水平均较治疗前均有显著改善(轻度窒息组t值分别为3.618、2.503、7.509;重度窒息组t值分别为3.284、3.481、8.558;均P<0.01);轻度窒息组血BUN、CR较治疗前均有显著改善(t值分别为5.208、4.247,均P<0.05);重度窒息组血BUN、CR无明显改善(P>0.05)。结论窒息性肾损伤患儿血Cys-C、血及尿β2-MG水平与窒息程度及肾损伤程度密切相关,是评估预后的敏感指标。Objective To investigate the relationship between serum cystatin C ( Cys-C) and β2 microglobulin (β2-MG) levels in blood and urine with renal injury caused by neonatal asphyxia and prognostic value. Methods From July 2013 to June 2014 68 neonates with renal injury caused by asphyxia were admitted in Liuzhou City Liutie Center Hospital, and they were divided into mild asphyxia group ( n=32) and severe asphyxia group (n=36) according to severity of disease. They were given routine symptomatic treatment. Another 30 healthy newborns were randomly selected in control group. The levels of Cys-C, serum β2-MG, urinary β2-MG, blood urea nitrogen ( BUN) and creatinine ( CR) of three groups were detected within 24 hours and 3 days after birth. Results Blood Cys-C, bloodβ2-MG and urinary β2-MG levels of mild asphyxia group were significantly higher than those of the control group ( t value was 7. 213, 6. 529 and 9. 006, respectively, all P〈0. 01), so were severe asphyxia group (t value was 9. 586, 9. 087 and 14. 189, respectively, all P〈0. 01). BUN and CR levels of severe asphyxia group were obviously higher than those of the control group and mild asphyxia group ( t value was 8. 956, 7. 323, 7. 634 and 6. 373, respectively, all P〈0. 01). The levels of blood Cys-C, bloodβ2-MG and urinaryβ2-MG of asphyxia groups were improved after treatment ( tmild group value was 3. 618, 2. 503 and 7. 509, tsevere group value was 3. 284, 3. 481 and 8. 558, respectively, all P〈0. 01). The mild asphyxia group had improvement in BUN and CR after treatment (t value was 5. 208 and 4. 247, respectively, both P〈0. 05), but severe asphyxia group did not (P〉0. 05). Conclusion Blood Cys-C, blood and urineβ2-MG levels are closely related with neonatal asphyxia renal injury and the severity of injury, which can be considered as sensitive indexes to evaluate prognosis.
