摘 要:目的评价采用三维超声的自由解剖成像(Omni View)结合容积对比成像(VCI)技术对子宫内膜息肉的诊断价值。方法对89例经阴道二维超声(2D-TVS)疑有子宫内膜病变的患者均进一步行传统经阴道三维超声(3D-TVS)、Omni View结合VCI技术的经阴道三维超声(Omni View结合VCI 3D-TVS)检查,再行宫腔镜检查,以病理结果为确诊依据,对3种超声检查方法的结果进行对比分析。结果在89例患者中,子宫内膜息肉62例,子宫内膜增生症17例,黏膜下肌瘤8例,子宫内膜癌2例。2D-TVS、3D-TVS、Omni View结合VCI 3D-TVS对子宫内膜息肉病变诊断的敏感性分别为80.64%、91.94%和98.39%差异有统计学意义(x^2=12.35,P=0.02);特异性分别为81.48%、92.59%和96.30%,差异也有统计学意义(x^2=3.54,P=0.03)。结论运用Omni View结合VCI 3D-TVS与2D-TVS及传统3D-TVS相比,对子宫内膜息肉具有更高的诊断价值,可作为早期诊断该疾病的超声新方法 。Objective To evaluate the diagnostic value of omni view combining volume contrast imaging ( VCI ) in 3-dimensional ultrasonography on endometrial polyp. Methods Eighty-nine patients suspected with endometrial polyp screened by traditional 2-dimensional transvaginal ultrasound scanning (2D-TVS) adopted traditional 3-dimensional transvaginal ultrasound scanning (3D-TVS) and omni view combined with VCI 3D-TVS. Then hysteroscopy examination was performed to detect endometrial polyp. Pathologic results were taken as definite diagnosis to compare the diagnosis of three methods. Results Among 89 patients, there were 62 cases with endometrial polyp, 17 cases with endometrial hyperplasia, 8 cases with submucosal myoma, and 2 cases with endometrial carcinoma. The sensitivity of 2D-TVS, 3D-TVS, and omni view combining VCI 3D-TVS was 80. 64%, 91. 94% and 98. 39%, respectively with significant difference (χ2 =12. 35,P=0. 02), and the specificity of them was 81. 48%, 92. 59% and 96. 30%, respectively. There were statistical differences among three methods (χ2 = 3. 54,P = 0. 03). Conclusion Omni view combining VCI 3D-TVS obtains higher diagnostic value on endometrial polyp than traditional 2D-TVS and 3D-TVS, so it can be taken as a new ultrasound technology to diagnose endometrial polyp.
