阿替普酶联合他汀类药物治疗急性缺血性脑卒中60例临床评价

摘　　要：目的观察重组组织型纤溶酶原激活剂(rt PA)联合他汀类药物治疗急性缺血性脑卒中对血管再通及预后的临床疗效。方法采用随机数字表法将120例4.5 h内急性缺血性脑卒中患者均分为两组,各60例。对照组给予瑞舒伐他汀钙片治疗,试验组在此基础上联合阿替普酶治疗。结果试验组总有效率(96.67%)明显高于对照组(70.00%);试验组治疗后患者的总胆固醇为(3.66±1.78)mmol/L,低密度脂蛋白胆固醇为(3.18±1.46)mmol/L,显著低于对照组的(5.54±1.79)mmol/L和(3.39±1.74)mmol/L;试验组患者治疗后美国国立卫生院神经功能缺损评分(NIHSS)1 h(19.0±6.31)分,24 h(15.3±5.29)分,3 d(9.6±3.01)分,1周(4.0±1.76)分,2周(1.8±0.36),均明显优于对照组患者的(23.0±6.65)分、(17.4±5.31)分、(15.0±4.45)分、(9.0±3.55)分和(5.6±2.12)分(P<0.05)。结论 rt PA联合他汀类药物综合治疗急性缺血性脑卒中具有良好的治疗效果,可显著调节血脂,保护神经系统功能。Objective To observe the clinical effect of recombinant tissue plasminogen activator（ rt PA） combined with statin drugs on the vascular recanalization and prognosis in the treatment of acute ischemic stroke. Methods 120 cases of acute ischemic stroke within4. 5 h after onset were divided into two groups by adopting the random number table method,60 cases in each group. The control group was treated with Rosuvastatin Calcium Tablet, while on this basis the experimental group was combined with rt PA. The clinical effects after treatment were compared between the two groups. Results The total effective rate after treatment in the experimental group was96. 67%,which was significantly higher than 70. 00% in the control group,the TC level and LDL- C level in the experimental group were（ 3. 66 ± 1. 78） mmol / L and（ 3. 18 ± 1. 46） mmol / L respectively, which were significantly lower than（ 5. 54 ± 1. 79） mmol / L and（ 3. 39 ± 1. 74） mmol / L in the control group,the National Institutes of Health Stroke Scale（NIHSS） score after treatment in the experimental group was（ 19. 0 ± 6. 31） points for 1 h,（ 15. 3 ± 5. 29） points for 24 h,（ 9. 6 ± 3. 01） points for 3 d,（ 4. 0 ± 1. 76） points for1 week and（ 1. 8 ± 0. 36） points for 2 weeks,all were more significant than those in the control group, which were（ 23. 0 ± 6. 65） points,（ 17. 4 ± 5. 31） points,（ 15. 0 ± 4. 45） points,（ 9. 0 ± 3. 55） points and（ 5. 6 ± 2. 12） points（ P〈0. 05）. Conclusion rt PA combined with statin drugs in treating acute ischemic stroke has a good therapeutic effect,can significantly decrease the blood lipids and protect the nervous system function.
