摘 要:目的观察HBV慢性感染携带者和慢性乙型肝炎患者的血小板参数及CD62P的变化规律。方法采用流式细胞术(FCM)测定HBV携带者、慢性乙型肝炎患者外周血中血小板膜糖蛋白CD62P的阳性表达率,同时对其血小板参数进行检测,并与55例健康对照组进行比较分析。结果 HBV携带者、慢性乙型肝炎患者体内血小板活化标志物CD62P的表达率分别为(4.00±0.93)%、(15.36±6.32)%,较健康对照组的(1.40±0.55)%显著升高,差异均有统计学意义(P<0.01),且慢性乙型肝炎患者体内血小板活化水平较HBV携带者显著升高(P<0.01);各组组间比较差异有统计学意义(F=389.6,P<0.01)。HBV携带者、慢性乙型肝炎患者血小板(PLT)均低于健康对照组(P<0.05),平均血小板体积(MPV)均高于健康对照组(P<0.05),HBV携带者、慢性乙型肝炎患者间PLT、MPV差异无统计学意义(P>0.05);各组间血小板比容(PCT)、血小板体积分布宽度(PDW)和大血小板比率(P-LCR)差异无统计学意义(P>0.05)。结论高表达的CD62P可能参与慢性乙型肝炎的发生和发展,联合PLT与MPV的测定可作为病毒性肝炎疾病进展及疗效观察的指标。Objective To observe the changing pattern of platelet parameters and CD62 P expression in HBV carriers and chronic hepatitis B patients. Methods The positive percentage of CD62 P on platelets membrane were analyzed by flow cytometry(FCM) and the value of platelet parameters were detected in HBV carriers and chronic hepatitis B patients, and compared with 55 cases of normal controls. Results The positive rates of CD62 P in HBV carriers and chronic hepatitis B patients were significantly higher than that of the normal controls(P〈0.01); the positive rates of CD62 P in chronic hepatitis B patients were significantly higher than that of the HBV carriers(P〈0.01); platelet(PLT) of HBV carriers and chronic hepatitis B patients were lower than that of the normal controls(P〈0.05); mean platelet volume(MPV) of HBV carriers and chronic hepatitis B patients were higher than that of the normal controls(P〈0.05), PLT and MPV were not significantly different between HBV carriers and chronic hepatitis B patients(P〉0.05); platelet hematocrit(PCT),platelet volume distribution width(PDW) and platelet large cell ratioc(P-LCR) had no significant differences among different groups(P〉0.05). Conclusions High expression of CD62 P may participate in the occurrence and development of chronic hepatitis. Combined detection of PLT and MPV can be used for predicting viral hepatitis disease progression and the curative effect.
