摘 要:目的了解0-8月龄婴儿胎传麻疹抗体随年龄增长的变化规律,为降低低月龄婴儿的麻疹发病率提供科学依据。方法以韶关市妇幼保健院和南雄市人民医院作为采样医院,采用随机抽样的方法抽取未患过麻疹也未注射麻疹减毒活疫苗的8月龄以下健康婴儿作为研究对象,分为0-、1-、2-、3-、4-、5-、6-、7-月龄共8个组,其中0-1月龄组采集100份脐带血标本,其余月龄组采集50份静脉血标本,分离血清后采用ELISA法进行麻疹Ig G抗体测定。若6-、7-月龄的婴儿依然麻疹Ig G抗体阳性,则采集其母亲的静脉血标本进行麻疹Ig G抗体测定。结果本研究共检测标本450份,其中阳性标本数202份,阳性率44.90%;阳性率最高的是0-1月龄组,阳性率为88.00%,平均几何浓度(GMC)为(1 273.11±25.32)m IU/ml;随月龄增加抗体水平迅速下降(χ2=412.35,P=0.000),到5-月龄时阳性率仅为12%,GMC(87.03±6.42)m IU/ml,低于阳性判断标准;6-、7-月龄时阳性率已经下降到6.00%,GMC分别为(91.86±5.27)m IU/ml和(84.95±15.24)m IU/ml,均低于阳性判断标准。对6例6-、7-月龄时依然麻疹Ig G抗体阳性者的母亲进行检测,其麻疹Ig G抗体水平最高1 472.33 m IU/ml,最低827.56 m IU/ml。结论胎传麻疹抗体随着年龄的增加迅速下降,初次麻疹疫苗免疫前基本失去保护作用,通过强化免疫提高育龄妇女的麻疹抗体水平,进而提高婴儿胎传麻疹抗体水平来预防初免前感染麻疹是较为有效的方法。Objective To study the changing levels of measles antibody titers in 0 -8 months old infants with age,and provide scientific basis for reduce measles incidence of infants. Methods Samples were collected from 0-8 months old infants had no measles and had no live attenuated measles vaccination from MCH and Nanxiong City People's Hospital hospital using random sampling method, and divided into 0 to 1, 1 to 2, 2 to 3, 3 to 4, 4 to 5, 5 to 6, 6 to 7, and 7 to8 months old age groups. 100 cord blood samples were collected from 0-1 month age group, and 50 blood samples from each age group. Measles Ig G antibody level was determined by ELISA. If the measles Ig G antibody was positive in infants of 6-7 to age, the blood samples of the venous blood samples collected for measles Ig G antibody were collected. Results In this study, 450 samples were detected, of which 202 were positive, with a positive rate of 44.90%; the highest positive rate was 0-1 month group, with the positive rate of 88.00% and GMC concentration of(1 273.11±25.32)m IU / ml; the antibody levels decline with increase in age(trend test:χ2=412.35,P=0.000). The positive rate of the 5 to 6 months old age group was dropped to 12%, with and average GMC(87.03±6.42) m IU / ml, below the positive criteria; the positive rate of the 6 to 7 and 7 to 8 months old age groups had been reduced to 6.00%, with GMC of(91.86±5.27)m IU / ml and(84.95±15.24) m IU / ml, respectively, lower than the positive standard.The measles antibodies of 6 cases of 6 to 8 months of age were detected, the measles Ig G antibody level was the highest and the lowest was 1 472.33 m IU / ml and 827.56 m IU / m.Conclusions Fetal transmission of measles antibodies decreased rapidly with increasing age. The infants lost measles protection before the first measles vaccination. Strengthening the immunity and boost up the measles antibody levels in women of childbearing age should be effective way to increase the measles antibody levels and protect against measle infection o
