累及食道黏膜的大疱性类天疱疮1例

摘　　要：患者男,77岁。周身皮肤红斑、水疱伴瘙痒2个月加重半个月。治疗第7天,进食后突感胸骨后疼痛,继之恶心、呕吐出长约35cm白色有弹性的条索样黏膜状物,表面粘附少许血液,组织病理示:分化成熟的鳞状上皮组织,见念珠菌菌丝、孢子、菌落及炎性渗出物。诊断:大疱性类天疱疮;剥脱性食管炎。A 77-year-old male presented with skin erythema, blisters with itching for 2 month and aggravated for 15 days. On the seventh days after treatment, the patient felt retrosternal pain suddenly after lunch,and then fol- lowed by vomiting. A 35cm long white elastic cord with a little blood ,which like the appearance of esophageal mucosa was seen after vomiting. Histopathology examination revealed that differentiated squamous epithelium withthe candida hyphae, spores, bacterial colonies and inflammatory exudate. It was diagnosed with bullous pemphigoid with exfoliative esophagitis.
