平乐正骨手法复位配合支具动态治疗发育性髋关节脱位50例

摘　　要：目的观察采用平乐正骨手法复位配合支具固定动态治疗发育性髋关节脱位患儿的临床疗效。方法选择河南省洛阳正骨医院50例60髋采用平乐正骨手法复位配合支具固定治疗发育性髋关节脱位并进行平均18个月的随访研究,观察4个治疗阶段的X线片的髋臼指数(acetabular index AI)和髋臼深度与宽度比值(acetabular index of depth to widt AI(D/W))的动态变化。结果四个阶段坚持治疗50例60髋AI由治疗前平均(36.32±2.01)°减少到(24.58±3.28)°,平均每月减少0.65°;AI(D/W)平均由治疗前平均(13.20±4.58)%增加到(24.20±3.50)%,平均每月增加0.61%。AI的回复过程和AI(D/W)值的增加过程皆优于健侧的生理反馈。结论认为在坚持平乐正骨动静结合、筋骨互横、内外兼治、医患合作的理论基础上采用平乐正骨手法复位配合支具对发育性髋关节脱位治疗可获得较好的疗效。Objective To observe the clinical curative effect of Pingle orthopedic manipulative reduction combined with dynamic brace fixation in the treatment of developmental dislocation of the hip in children. Methods 50 cases of 60 hips in Luoyang Orthopedic Hospital by Pingle orthopedic manipulative reduction combined with brace fixation for the treatment of developmental dislocation of the hip and followed up 18 months on the average. Acetabular index X-ray 4 stages of treatment（ acetabular index AI） and acetabular depth and width ratio（acetabular index of depth to widt AI（D/W）） of the dynamic changes were observed. Results 4 stages on treatment of 50 cases of 60 hips by AI before treatment the average（36.32 ＋ 2.01） was reduced to（24.58 ＋ 3.28） degrees, average monthly decline of0.65 degrees; AI（D/W） by the average before treatment an average of（13.20 ＋ 4.58） % to（24.20 ＋ 3.50） %, the average monthly increase of 0.61%. The recovery process of AI（D/W） value of the process is better than that of the contralateral physiological feedback.Conclusion We thought that on the basis of insisting on combination of dynamic and static, internal and external therapy, mutual cross doctor-patient cooperation, adopted Pingle orthopedic manipulative reduction combined with brace dynamic in the treatment of developmental dislocation of the hip can obtain good effect.
