摘 要:目的探讨斑点追踪显像技术测量二尖瓣环位移(MAD)评价心衰(HF)患者左心室收缩功能的临床应用价值。材料与方法心衰患者43例,依据射血分数将其分为射血分数正常组(HFNEF)25例和射血分数减低组(HFREF)29例,另选健康对照组29例。采用Philips iE33型超声诊断仪,在心尖四腔心切面以Simpson法测量左心室射血分数(LVEF),将心尖四腔心切面的动态图像输入Qlab6.0工作站,获得胸壁处室间隔瓣环插入点最大位移(TMAD1),左心室侧壁瓣环插入点最大位移(TMAD2),室间隔-侧壁瓣环点连线中点的最大位移(TMAD Midpt)及其占左心室长径的百分数即左心室长轴缩短率(TMAD Midpt%)等参数,并进行比较。结果对照组、HFNEF组及HFREF组LVEF逐渐减低(均P<0.05);心衰各组的MAD各测值均显著低于对照组,且HFREF组MAD各测值均亦显著低于HFNEF组(均P<0.01);MAD各参数与LVEF均呈显著正相关,r值分别为0.73(17MAD1),0.71(17MAD2),0.74(TMADMidpt),0.78(TMADMidpt%)(均P<0.01)。结论斑点追踪显像技术测量MAD能简单、无创评价心衰患者左心室收缩功能。结论:SPTP多见于年轻女性,CT和MRI表现具有一定的特征性。Purpose To explore the clinical value by mitral annular displacement (MAD) measured by speckle tracking imaging technique for assessing left ventricular global systolic function in patients with heart failure (HF). Materials and Methods 43 patients with heart failure, these patients were divided into 25 patients with normal ejection fraction and 29 pa- tients with produce ejection fraction according to the ejection fraction, as well as 29 healthy subjects were enrolled in this stud- y. Left ventricular ejection fraction (LVEF) was measured by Simpson method at the apical four-chamber view by using Phil- ips iE33 echocardiography. When one point was placed at the chest wall, the maximal displacement at interventricular septum (TMAD1), the maximal displacement at left ventricular lateral wall (TMAD2), the maximal displacement at the midpoint of interventricular septum and left ventricular lateral wall (TMAD Midpt), and the left ventricular longitudinal shortening (TMAD Midpt% ) were obtained using off-line Qlab 6. 0 software. Campared with these parameters. Results LVEF was de- creasing in control group, HFNEF and HFREF group( all P 〈 0. 05 ) ; The parameters of MAD were significantly lower in pa- tients with heart failure group than in control group, and The parameters of MAD were significantly lower in HFREF group than in HFNEF group ( all P 〈 0.05 ). The parameters of MAD correlated positively with LVEF, r = 0.73 ( TMAD1 ), 0. 71 (TMAD2), 0. 74(TMAD Midpt), 0. 78(TMAD Midpt% ), respectively(all P 〈0. 05 ). Conclusion Based on speckle tracking imaging technique, MAD is a novel, non-invasive parameter which may be used in detection of left ventricular systolic function in patients with heart failure.
