应用InterTan与PFNA治疗老年患者股骨粗隆间骨折疗效分析

摘　　要：目的探讨老年股骨转子间骨折两种髓内固定方法（Inter Tan与PFNA）的临床效果。方法回顾2011年~2012年在我院收治的老年股骨粗隆间骨折患者中,随机选取应用两种内固定方法治疗病例60例,其中33例应用Inter Tan治疗,27例应用PFNA治疗。结果 60例患者均得到随访,时间12~28个月,平均18.5个月,无死亡个案,Inter Tan组：手术时间为（65.2±15.5）min,术中出血量为（261.5±110.7）ml,下地行走时间为（5.3±0.8）周,骨折愈合时间为（8.8±1.3）周;PFNA组：手术时间为（40.4±16.1）min,术中出血量为（192±120.6）ml,下地行走时间为（5.0±0.6）周,骨折愈合时间为（8.6±0.8）周。在手术时间及术中出血量方面,两组差异有显著性意义,在下地行走时间及骨折愈合时间方面没有差异。结论 Inter Tan及PFNA均是治疗老年患者股骨粗隆间骨折的有效方法,但应用PFNA方法的病例,手术时间短,术中出血量更少。Objective To compare the clinical results of the intramedullary nails（Intertan and the proximal femoral nail-antirotation） in treatment of intertrochanteric fractures of femul. Methods 60 elderly patients with intertrochanteric fracture of femur were randomized into 2 treatmentgroups： 33 cases were treated with Intertan and 27 cases with PFNA in our hospital between 2011 and 2012. Results All cases were followed up for 12 to 28 months. No one died.in the Intertan group, the mean time for operation was（65.2±15.5） minutes, the mean blood loss during operation was（261.5±110.7） ml, the mean time for walking after operation was（5.3±0.8） weeks and the mean time for bone union was（8.8±1.3） weeks. In the PFNA group, the mean time for operation was（40.4±16）. 1minutes,the mean blood loss during operation was（192±120.6） ml, the mean time for walking after operation was（5.0±0.6） weeks and the mean time for bone union was（8.6±0.8） weeks. There were significant differences in the mean time for operation and the mean blood loss between the 2 groups（P〈0.05）. Conclusion Both the Intertan and the proximal femoral nail-antirotation are reasonable choices for treatment of the intertrochanteric fractures of femur in elderly patients. but the operation time is longer and the blood loss is more in use of the Intertan.
