不同剂量地佐辛预防舒芬太尼诱发呛咳反应的临床观察

摘　　要：目的观察和评价不同剂量地佐辛对全身麻醉诱导过程中舒芬太尼诱发呛咳反应的预防作用。方法选取择期全身麻醉手术患者240例，美国麻醉医师协会（ASA）分级Ⅰ-Ⅱ级，年龄18岁～65岁，男女不限，采用随机数字表法分为3组（每组80例）：对照组（c组）、地佐辛1组（D1组）、地佐辛2组（D2组）。全身麻醉诱导静脉注射舒芬太尼前3min（T0），C组、D1组和D2组分别静脉注射生理盐水5ml、地佐辛0．05mg／kg和地佐辛0．10rag／ks。诱导开始（T1）静脉注射舒芬太尼（0．5μg/kg），随后2min内观察并记录患者呛咳发生率、次数、心率（heartrate，HR）、血压和脉搏血氧饱和度（pulse oxyge nsaturation，sp02）。结果D2组患者均未发生呛咳，与C组35例发生呛咳（轻度11．3％，中度12．5％，重度20．0％）和D1组32例发生呛咳（轻度13．8％，中度8．8％，重度17．5％）比较，差异有统计学意义（P〈0．01）。D1组和C组呛咳发生率和严重程度差异无统计学意义（P〉0．05）。结论预注0．10ms／ks地佐辛能安全有效地抑制舒芬太尼诱发的呛咳反应。Objective We aimed to evaluate the efficacy of dezocine in preventing sufentanil-induced cough during induction stage of general anesthesia. Methods Two hundred and forty patients （ASAⅠ -Ⅱ ）, aged 18-65, weighing 40 kg-80 kg, male or female, undergoing elective surgery were randomly divided into control group （group C）, dezocine group 1 （group D1） and dezocine group 2 （group D2）, 80 patients in each group. Patients in the control group and the experiment group （D1-D2） separately received 5 ml normal saline, dezocine at 0.05 mg/kg and 0,10 mg/kg 2 min before intravenous infusion of sufentanil（0.5μg/kg）. The incidence and severity of cough in patients were evaluated 2 min after the injection of sufentanil in the anesthesia induction period. Results No patient had cough in group D2, 35 patients in group C had cough （severity of cough：mild, 11.3%, moderate, 12.5%, severe, 20.0%） and 32 patients in group D1 had cough （severity of cough： mild, 13.8%, moderate, 8.8%, severe, 17.5%）. The occurrence and reflex degree of cough in group D2 was significantly lower than that in group C and group D1 （P〈0.01）, while there was no significant difference between group C and group D1. Conclusions The results of current study suggest that administration of dezocine 0.1 mg/kg may effectively prevent the sufentanil-induced bucking in patientswith stable hemodynamicsduring general anesthesia induction.
