羟氯喹联合丙种球蛋白治疗老年性肾病综合征的效果观察

摘　　要：目的探讨羟氯喹联合丙种球蛋白治疗老年性肾病综合征的临床效果。方法选取2010年6月~2014年5月在本院的118例老年性肾病综合征患者作为研究对象,随机分为研究组和对照组,各59例。两组均给予激素治疗,对照组在此基础上给予丙种球蛋白,研究组在对照组的基础上给予羟氯喹。比较两组治疗前后的24 h尿蛋白定量、肌酐、尿素氮、血白蛋白、丙氨酸氨基转移酶水平及不良反应发生率。结果研究组治疗后的24 h尿蛋白定量、肌酐水平显著低于对照组治疗后,差异有统计学意义（P〈0.05）。研究组治疗后的尿素氮、丙氨酸氨基转移酶水平显著低于对照组治疗后,差异有统计学意义（P〈0.01）。研究组治疗后的血白蛋白水平显著高于对照组治疗后,差异有统计学意义（P〈0.01）。两组的不良反应发生率比较,差异无统计学意义（P〉0.05）。结论羟氯喹联合丙种球蛋白治疗老年性肾病综合征的效果显著,且无严重不良反应,值得临床推广应用。Objective To explore the clinical effect of hydroxychloroquine combined with gamma globulin in the treatment of senile nephrotie syndrome. Methods 118 patients with senile nephrotic syndrome from June 2010 to May 2014 in our hospital were selected and randomly divided into the research group and the control group,59 cases in each group.All patients were given hormone therapy.The control group was given gamma globulin on the basis of hormone therapy.The research group was given hydroxychloroquine on the basis of the control group.The level of 24 h urine pro- rein,serum creatinine,urea nitrogen,blood albumin, alanine aminotransferase and the incidence rate of adverse reaction before and after treatment in two groups was compared respectively. Results The level of 24 h urine protein,serum creatinine after treatment in the research group was lower than that in the control group after treatment,with significant difference（P〈0.05）.The level of urea nitrogen and alanine aminotransferase after treatment in the research group was lower than that in the control group after treatment,With significant difference （P〈0.01）.The level of blood albumin after treatment in the research group was higher than that in the control group after treatment,with significant difference（P〈0.01）. The incidence rate of adverse reaction compared two groups,there was no significant difference （P〉0.05）. Conclusion Hydroxychloroquine combined with gamma globulin in the treatment of senile nephrotic syndrome has favorable effect, and no severe adverse effects occur,it is worthy of clinical promotion and application.
