微小切口PFNA治疗老年股骨粗隆间骨折的效果分析

摘　　要：目的探讨微小切口PFNA治疗老年股骨粗隆间骨折的临床疗效。方法选取2011年2月~2013年2月于本院就诊老年股骨粗隆间骨折患者共46例,将其随机分为观察组和对照组,各23例,对照组患者入院之后进行DHS治疗;观察组患者进行微创PFNA治疗,并将大粗隆顶点定位开口方法给予改进,将切口缩小到2 cm,创伤显著降低。比较两组患者的治疗效果。结果观察组患者治疗良好率为91.3%,明显高于对照组的47.8%,观察组患者术后并发症的发生率为13%,低于对照组的69.6%。结论微小切口PFNA治疗老年股骨粗隆间骨折疗效显著,手术操作简单、创伤小、时间短、恢复时间短,值得临床推广应用。Objective To explore the clinical effect of applying micro-incision proximal femoral nail anti-rotation （PFNA） to treat elderly intertroehanterie fractures. Methods From February 2011 to February 2013,46 elderly patients with intertroehanterie fractures visited our hospital for treatment were selected and evenly divided into two groups in random.In control group,dynamic hip screw （DHS） was provided after being admitted,while in observation group,micro-incision PFNA was adopted.The position opening of vertex of greater trochanter was improved,the incision was shortened group.The incidence of postoperative complications in observation group was 13%,while the incidence in control group was 69.6%.The incidence of complications in observation group was greatly lower than that in observation group. Conclusion Micro-incision PFNA can obtain a remarkable effect on treating elderly intertrochanteric fractures,at advantages of simple and easy in operation,small wound,short oeriod.and quick recovery,which is worthy of promotion in clinic.
