摘  要:目的 探讨外源性硫化氢(H2S)对心跳骤停复苏大鼠脑组织炎症反应及生存率的影响。方法 健康雄性SD大鼠90只,体重260-280g。按随机原则分为三组:对照组(Sham组,n=20):吸入麻醉药物诱导,气管插管,动静脉置管,但不进行心跳骤停及复苏操作。其余大鼠经气管插管后进行窒息性心跳骤停并复苏,自主循环恢复(ROSC)后随机分为两组:心跳骤停复苏组(CA组,n=35)、H2S组(n=35)。H2S组大鼠复苏恢复自主循环(ROSC)后立即吸入80ppm H2S 60min。每组取15只大鼠,于ROSC后(sham组麻醉后)3、24和72h时断头取脑组织,测定脑组织中白细胞介素10(IL-10)、白细胞介素1β(IL-1β)、肿瘤坏死因子α(TNF-α)及髓过氧化物酶(MPO)的含量。采用胶带移除实验评估大鼠ROSC后第14d的神经功能。记录ROSC后14d内3组大鼠的存活数量,随后断头取脑,计数海马CA1区存活神经元个数。结果 ROSC后3h时H2S组IL-10表达量明显高于Sham和CA组(P〈0.05);CA和H2S组在ROSC后24h时脑组织IL-1β表达量,ROSC后3、24h时脑组织TNF-α表达量明显高于Sham组,且CA组明显高于H2S组(P〈0.05);ROSC后72h时CA组MPO活性明显高于Sham组和H2S组(P〈0.05)。CA组及H2S组存活神经元数量明显少于Sham组,且CA组明显少于H2S组(P〈0.05)。ROSC后14dSham和H2S组存活率明显高于CA组(P〈0.05)。结论 大鼠心跳骤停复苏后早期吸入外源性80ppm H2S可提高心肺复苏后14d生存率,其机制可能与抑制脑组织炎症反应,改善海马CA1区神经元存活有关。Objective To investigate the effects of exogenous hydrogen sulfide(H2S)on neuroinflammation and survival rate in a rat model of asphyxial cardiac arrest and resuscitation.Methods Asphyxial cardiac arrest and resuscitation were performed in Sprague-Dawley rats.After return of spontaneous circulation(ROSC),rats were randomly divided into 2groups:cardiac arrest group(group CA,n=20)and group H2S(n=20).Rats in groups CA and H2 S inhaled 30% O2or30% O2 containing 80ppm H2 S for 60 min after ROSC.Rats in control group(group Sham,n=5)underwent all procedures except cardiac arrest and resuscitation.The levels of interleukin-10(IL-10)、interleukin-1β(IL-1β)、tumor necrosis factor-α(TNF-α)and the activities of myeloperoxidase(MPO)were measured at 3h,24 h,72hafter ROSC.At 14 dafter ROSC,tape removal test was conducted to evaluate neurologic outcome,survival rates in all groups were recorded and then rats were sacrificed and examined for the viable neurons in hippocampus CA1 region.Results At 3hafter ROSC the levels of IL-10 in group H2 S was obviously higher than that in groups sham and CA(P〈0.05);At 24 h after ROSC the levels of IL-1β,at 3,24 hafter ROSC the TNF-αexpression in group H2 S was significantly higher than that in groups Sham and CA,and that in group CA was significantly higher than that of group H2S(P〈0.05);At 72 hafter ROSC MPO activity in group CA was significantly higher than that in groups Sham and H2 S group(P〈0.05).The number of viable neurons in groups CA and H2 S was significantly less than that of group Sham,and that in group CA was significantly less than that in group H2S(P〈0.05).The survival rates of groups Sham H2 S at 14 dafter ROSC were significantly higher than those of group CA(P〈0.05).Conclusion Inhalation of 80 ppm H2S at the early stage can improve the survival rate in rats after cardiac arrest and resuscitation.The inhibition of neuroinflammatory response and improvement of neuron survival in the hippocampus CA1 regi
