输血管理体系指导医院合理用血的作用探讨

摘　　要：目的:探讨输血管理体系对临床合理用血的作用。方法:选取2013年7月-12月1 400份采用输血管理体系的住院输血病历为观察组,并选2012年同期1 400份未采用输血管理体系的住院输血病历为对照组,比较两组输血病历不合理用血情况。结果:观察组输血病历较对照组住院患者人均输红细胞量下降了34.4%、人均输血浆量下降了67.5%、人均输血小板下降了49.2%及住院手术患者人均输血量下降了14.4%;人均输冷沉淀升高了45.2%;重点用血科室住院患者人均输红细胞量和血浆量明显下降,其中ICU、胃肠外科和肝胆外科下降率达50%以上;输血适应症符合率提高了17.6%;红细胞不合理输注下降55.6%;血浆不合理输注下降了44.6%;血小板不合理输注下降40.9%,冷沉淀不合理输注下降75%。结论:输血管理体系对临床合理用血发挥了关键性指导作用。Objective:To discuss role of blood transfusion management system in guidance of rational use of blood. Methods:Two groups were selected:observation group (n=1400, blood transfusion records from July to December, 2013, after the application of blood transfusion management system), and control group (n=1400, blood transfusion records from July to December, 2012, before the application of blood transfusion management system) . The unreasonable applications of blood of observation group and control group were compared. Results:After the application of blood transfusion management system, the per capita quantities of red blood were de-creased by 34. 4%, amounts of plasma were decreased by 67. 5%, quantities of platelet were decreased by 49. 2%, volumes of opera-tion inpatients were decreased by 14. 4%, and the per capita amounts of cryoprecipitate injection were increased by 45. 2%. At the same time, the per capita quantities of red blood and plasma of key departments were significantly decreased, wherein, those of ICU, gastrointestinal surgery and department of hepatobiliary surgery were declined by more than 50%;the coincidence rates of the indica-tions for transfusion rates were increased by 17. 5%;the unreasonable infusion rates of red blood, plasma, platelet, and cryoprecipitate were decreased by 55. 6%, 44. 6%, 40. 9%, and 75% of observation group than control group. Conclusions:The blood transfusion management system plays a key role in the guidance of the clinical rational use of blood.
